以CK19为标记物研究pN0胃窦癌区域淋巴结微转移

摘　　要：目的通过检测常规病理检查结果为全部阴性的胃窦癌淋巴结CK19的表达量,证实pN0胃窦癌区域淋巴结中存在微转移,分析微转移与胃窦癌临床病理学的相关性。方法取20例pN0胃窦癌患者经常规病理检测认为无转移的区域淋巴结蜡块99个,5例非肿瘤患者淋巴结蜡块19个,用RT-PCR检测淋巴结中K19mRNA表达,使用统计学方法分析检测结果与患者临床病理因素的相关性。结果20例胃窦癌患者中,有5例患者(25%)共计21个淋巴结(21.2%)检测阳性,5例非肿瘤患者淋巴结中均未检测到CK19mRNA。pN0胃窦癌淋巴结CK19mRNA阳性表达与肿瘤的TNM分期呈正相关性(P<0.05),与肿瘤的部位、大小、分化程度以及患者的性别、年龄无相关性(P>0.05)。结论PN0胃窦癌区域淋巴结存在微转移,淋巴结微转移与肿瘤浸润深度有关,CK19mRNA检测胃癌淋巴结微转移特异性高。Objective To detect CK19 of gastric antrum cancer lymph node by routine pathological examination and results were all negative. And to confirm micrometastasis was occurring in p N0 gastric antrum cancer lymph node and to explore the relevance between micrometastasis and clinical pathology of gastric antrum cancer. Methods 20 cases of p N0 gastric antrum cancer patients with regular pathological examination that there was no metastasis of lymph node paraffin block 99,five cases of non- tumor lymph node paraffin block 19,the expression of K19 m RNA was detected by using RT- PCR in lymph nodes. The correlation between the detection results and the clinical pathological factors were analyzed. Results In all of the 20 cases of gastric antrum cancer patients,there was five patients（ 25%） with total of 21 lymph nodes（ 21. 2%） tested positive and five non- tumor patients tested negative. Lymph node metastasis of gastric antrum carcinoma was related to tumor invasion and depth（ P〈0. 05）,and the age,sex,tumor size,degree of differentiation,and location of p N0（ P〉0. 05） were not related. Conclusion The regional lymph node of p N0 gastric antrum cancer present micrometastasis,and CK19 m RNA can be used as an ideal target gene for detection of gastric cancer with lymph node metastasis. Lymph node metastasis is related to the depth of tumor invasion.
