进展期大肠癌手术患者的希望水平及影响因素分析

摘　　要：目的探讨进展期大肠癌手术患者的希望水平,分析其影响因素。方法选择进展期大肠癌手术患者102例,采用《一般资料调查表》《Herth希望量表》《医学应对方式问卷》及《癌症患者生命质量量表》进行调查,对影响希望水平的因素采用Logistic逐步回归分析。结果男性明显多于女性,汉族多于回族;已婚、高中以上文化程度及人均月收入>5 000元组的患者,希望水平中对现实和未来的积极态度维度得分均高于离异、丧偶、初中以下文化程度及人均月收入<5 000元的患者组(P<0.05);临床病理分期中Ⅱ~Ⅲ期患者的希望水平各维度得分均高于Ⅳ期(P<0.05);无造口患者希望水平各维度得分均高于有造口的患者。Logistic回归分析显示,患者经济状况、总体健康状况、临床病理分期及屈服4个变量进入回归方程(r=0.479,R2=0.336)。结论总体健康状况、家庭经济状况、临床病理分期及屈服应对方式是影响进展期大肠癌手术患者希望水平的主要因素。Objective To explore the hope level of advanced colorectal cancer surgery patients and to analyze its influence factors. Methods 102 cases of patients with advanced colorectal cancer surgery was selected and the general information questionnaire was used for investigating the impact factors by Herth hope scale medical coping styles questionnaire,and analyzed the factors by Logistic regression analysis. Results Men were higher than women,and ethnic Han were higher than Hui in hope level; The reality and the future positive attitude scores of hope level in married,high school education or above and per capita monthly income high than 5000 RMBgroup were higher than the divorced,widowed,junior high school education and per capita monthly income less than 5 000 RMB group;Each dimension scores of hope in clinical pathologic stage Ⅱ- Ⅲ were higher than Ⅳ Period（ P〈0. 05）; Each dimension scores of patients without colostomy were higher than patients with colostomy. Logistic regression analysis showed that the patients with economic conditions,overall health condition,clinical pathologic staging and yield four variable into the regression equation（ r = 0. 479,r2= 0. 336）.Conclusion General health,family economic status,clinical staging and yield coping styles are the main factors affecting the level of advanced colorectal cancer surgery patients hope.
