多层螺旋CT在直肠癌术前T分期诊断中的价值

摘　　要：目的探讨多层螺旋CT(MSCT)对直肠癌手术前T分期诊断的准确性。方法搜集经肠镜、临床及其他方法断为直肠癌的143例病例资料,术前进行CT平扫及3期动态增强扫描检查。CT术前T分期与术后病理分期进行对照,评价CT术前T分期的价值。结果 MSCT对≤T2、T3、T4期直肠癌分期符合率分别为88.7%、81.45%、92.74%,敏感度分别为72.73%、85.14%、82.35%,特异度分别为94.51%、76%、94.39%;T分期总符合率为81.45%。结论 MSCT对直肠癌术前T分期有较高的准确性,对于直肠癌术前评估及治疗中有重要意义。Objective To explore the accuracy of multi- slice spiral CT（ MSCT） in the diagnosis of T- staging of rectal carcinoma before surgery. Methods The clinical materials of 143 cases with colonoscopy and other diagnostic methods for rectal carcinoma were collected. All the patients underwent preoperative plain and triphasic dynamic contrast enhanced MSCT. T- staging was evaluated and correlated with pathology. The value of CT preoperative staging was evaluated. Results The accuracy of MSCT for T- staging of T1- 2,T3 and T4 was 88. 7%,81. 45% and 92. 74%. The sensitivity was 72. 73%,85. 14% and 82. 35%. The specificity was 94. 51%,76% and 94. 39%. The whole accuracy was 81. 45%. Conclusion High accuracy preoperative staging could be obtained by MSCT,and it can provide information for surgery guidance and treatment.
