老年妇女高危型生殖道乳头瘤病毒感染情况分析

摘　　要：目的分析老年妇女高危型生殖道乳头瘤病毒(HPV)感染情况。方法选取2011年8月至2015年8月延安大学附属医院妇科门诊收治的老年妇女769例为研究对象。所有老年妇女均行HPV分型检测,再筛查宫颈细胞学,HPV阳性者继续检查病理组织,分析HPV感染情况。结果所有老年妇女中,总HPV感染率为12.0%(92/769)。其中高危型HPV(HR-HPV)和低危型HPV(LR-HPV)感染率分别为10.8%(83/769)、1.2%(9/769);单一感染率为10.0%(77/769),多重感染率为2.0%(15/769);其中,在多重感染中,二重感染率为1.4%(11/769);三重感染率为0.3%(2/769);四重感染率为0.1%(1/769);五重感染率为0.1%(1/769)。单一HPV感染前5位的为HPV16、52、58、6、18型,除HPV6外,其他均为HR-HPV。多重感染中HR-HPV合并高危型共12例,LR-HPV合并低危型共3例,分别占80.0%(12/15)和20.0%(3/15)。结论我院老年妇女HPV感染主要以单一感染为主,其合并感染以HR-HPV合并高危型为主。Objective To investigate the correlation between high-risk genital human papilloma virus （HPV） infection and cervical lesions of elderly women. Methods A total of 769 elderly women treated in Affiliated Hospital of Yah＇an University were included in the study. All women underwent HPV typing test, and then cervical cytology was done, pathological tissue examination was done, and the HPV infection of the three groups was compared. Results Among all of the elderly women,the total HPV infection rate was 12.0% （92/769）. The infection rate of HR-HPV and LR-HPV were 10. 8% （83/ 769 ）, 1.2% （ 9/769 ） respectively; single infection rate was 10. 0% （ 77/769 ） and multiple infection rate was 2. 0% （ 15/ 769） ; among which the double infection rate was 1.4% （11/769） ;triple infection rate was 0. 3% （2/769） ;fourfold infec- tion rate was 0. 1% （1/769） ;fivefold infection rate was 0. 1% （1/769）. According to the infection,the elderly women were divided into HR-HPV group and LR-HPV group. Top 5 single HPV infection were HPV16,52,58,6,18, except HPV6, others were all HR-HPV. There were 12 eases of multiple HR-HPV infection, 3 cases of LR-HPV, accounted for 80.0% （12/15） and 20. 0% （3/15）. Conclusion HPV infection in elderly women in our hospital is mainly single infection,and the combined infection is mainly HR-HPV.
