摘　　要：目的:探讨老年胃癌腹腔热灌注化疗联合静脉化疗治疗恶性腹水的临床疗效。方法:将92例老年胃癌合并恶性腹水患者随机分为两组。对照组46例给予静脉化疗联合腹腔抽液,而试验组46例采用腹腔热灌注化疗联合静脉化疗。比较两组临床疗效、不良反应的差异。结果:试验组临床总有效率为93.5%,显著高于对照组的76.1%,差异有统计学意义(P<0.05);试验组患者出现1例Ⅲ度骨髓抑制,试验组化疗后的不良反应较对照组稍重,差异无统计学意义(P>0.05)。结论:老年胃癌腹腔热灌注化疗联合静脉化疗治疗恶性腹水效果显著,其具有较高的临床有效率,不良反应也较轻,进而提高患者生存质量及治疗依从性,值得临床选择。Objective To study the clinical effect of hyperthermic intraperitoneal chemotherapy combined with intravenous chemotherapy in elderly gastric carcinoma patients complicated with malignant ascites. Method 92 elderly gastric carcinoma patients complicated with malignant ascites were randomly divided into two groups,46 cases each group. The control group adopted the intravenous chemotherapy combined with aspiration of abdominal fluid; the test group adopted the hyperthermic intraperitoneal chemotherapy combined with intravenous chemotherapy. The clincial effect and toxic and side effect for two groups were compared. Results The total effective rate of test group（93. 5%） was signficantly higher than that of control group（76. 1%）,the difference was statistically significant（P〈0. 05）; the test group had Ⅰ case of Type Ⅲ of bone marrow suppression; the toxic and side effect for test group was slighter than that of control group,the difference wasn＇ t statistically significant（P〉0. 05）. Conclusion For elderly gastric carcinoma patients complicated with malignant ascites,the hyperthermic intraperitoneal chemotherapy combined with intravenous chemotherapy has a higher clinical effective rate and slighter toxic and side effect,improves the patient＇s life quality and treatment compliance. It shall be recommended in clinical practices.
