摘  要:目的:观察痰瘀同治法改善咳嗽变异性哮喘(CVA)大鼠病症结合动物模型气道慢性炎症的疗效。方法:将72只大鼠分为正常对照组、模型对照组、激素吸入组、清肺化痰组、活血化瘀组、痰瘀同治组,制备咳嗽变异性哮喘痰瘀互结结合大鼠模型,灌胃给药15天,观察各组大鼠一般状态、中医证候、肺支气管病理组织形态、支气管肺泡灌洗液(BALF)嗜酸性粒细胞计数及测定血清Ig E水平的变化。结果:与正常对照组比较,模型组CVA大鼠支气管管腔狭窄,黏膜增厚,支气管壁及可见大量嗜酸性粒细胞浸润,BALF涂片EOS计数显著升高,血清中Ig E的含量显著升高,与正常对照组比较均有显著性差异(P<0.01)。激素治疗组及各中药治疗组,肺支气管病理、EOS计数及血清Ig E含量均有显著改善,与模型组比较有显著性差异(P<0.01),以激素治疗组及肝肺同治组指标改善最为显著。痰瘀同治法可以明显改善模型大鼠气滞血瘀、痰瘀互结的中医证候,改善大鼠一般状态。结论:痰瘀同治法可改善痰瘀互结的中医证候,降低CVA模型大鼠慢性气道炎症反应,调整免疫异常。Objective : To investigate the curative effect of treating both phlegm and blood stasis on CVA in rats. Meth- ods:The combination of disease and syndrome animal model of CVA rats was established by TCM etiology and western medicine etiology pathology and then they were randomly divided into normal group, model group, hormone inhaling group, clearing lung heat and eliminating phlegm group, activating blood and transforming stasis group, both phlegm and blood stasis treatment group. Observe the rats' general state, spontaneous activity, cough response, bronchial lung patholo- gy organization form, bronchoalveolar lavage fluid (BALF) eosinophil count and serum IgE. Results:Compare with normal rats,the bronchial lumen of the model group was stenosis, mucosal thickening, numerous eosinophils infiltration on bron- chial wall. The EOS counting was significantly increased in BALF smear and the content of serum IgE was significantly increased( P 〈 0. 01 ). Compared with model rats, bronchial lung pathology, the EOS counting and serum IgE content of hormone inhaling group and TCM treatment group were significantly increased( P 〈 0. 01 ), especially the hormone inhaling group and treating both phlegm and blood stasis group. Treating both phlegm and blood stasis could significantly improved the TCM syndrome of phlegm accumulating and blood stasis and general status in rats. Conclusion : Treating both phlegm and blood stasis could significantly improve the TCM syndrome of phlegm accumulating and blood stasis, reduce the chron- ic airway inflammation of CVA model rats and regulate the immune abnormalities.
