摘　　要：目的:分析不同类型原发性头痛发病的相关危险因素及与焦虑、抑郁精神障碍共病的现况。方法:连续选取自2013年8月至2015年12月我院门诊原发性头痛患者,记录性别、年龄、职业、教育程度、家族史、头痛类型等,结合视觉模拟评分法(VAS)和头痛影响测评量表-6(HIT-6)对头痛程度、生活质量进行评估,并采用汉密顿焦虑、抑郁量表进行测定。结果:共收集有效病例210例,女性占79%,其中紧张型头痛110例、无先兆偏头痛80例、先兆偏头痛20例,原发性头痛发病的高危因素包括:职业、文化程度、家族史及是否共患焦虑、抑郁(P<0.05)。紧张型头痛合并焦虑、抑郁最高,分别为46.4%、42.7%,其次是先兆型偏头痛,分别为45%、35%,无先兆偏头痛合并焦虑、抑郁分别为28.8%、25%。结论:门诊原发性头痛中,以紧张型头痛最常见,患者的职业、文化程度、家族史,是否共患焦虑、抑郁是其发病的高危因素。Objective： To analyze the risk factors for different types of primary headache and the comorbidities with anxiety and depression. Methods： Patients were enrolled from outpatient clinics of Aerospace Central Hospital between August 2013 and December 2015. Clinic data including gender, age, occupation, education level, family history and type of headache were recorded and analyzed. Using Visual Analogue Scale（VAS） and Headache Impact Test-6（HIT-6）, the headache degree and the quality of life were evaluated. The anxiety and depression scale were measured by Hamilton Rating Scale. Results： Two hundred and ten patients were enrolled（female accounted for 79%） including one hundred and ten of tension type headache（TTH）, eighty of migraine without aura（MOA） and twenty of migraine with aura（MA）. The risk factors for primary headache were occupation, educational level, family history and comorbidities with anxiety and（or） depression（P〈0.05）. The most common type of primary headache comorbid with anxiety and depression is TTH（46.4%, 42.7%）, followed by MA（45%, 35%） and MOA（28.8%, 25%）. Conclusion： TTH is the most common primary headache among out-patients. Independent risk factors are occupation, education level, family history and comorbidities with anxiety and（or） depression.
