摘 要:目的探讨超声引导下神经刺激仪在肌间沟入路臂丛神经阻滞临床麻醉教学的可行性与有效性。方法选择麻醉专业低年资住院医师(<3年)10名,按照随机数字表法分为试验组(超声引导下神经刺激法)和对照组(传统盲探法),每组各5名,每位医师为5位患者进行操作。2组医师分别进行2 h的麻醉前培训,将理论教学、多媒体教学、模拟教学与临床教学相结合。观察两组肌间沟入路臂丛神经阻滞麻醉时间、麻醉效果、并发症发生情况以及患者和住院医师满意度。结果与对照组比较,试验组肌间沟入路臂丛神经阻滞麻醉效果优,起效时间明显降低(P<0.05);患者以及住院医师满意度均明显增高(P<0.05)。结论临床教学中应用超声引导下神经刺激仪进行肌间沟入路臂丛神经阻滞可提高阻滞成功率,增加患者及住院医师满意度。Objective To explore the feasibility and effectiveness of introducing ultrasoundguided nerve stimulator into clinical anesthesia teaching for treatment of brachial plexus block. Methods Ten junior residents ( 〈3 years ) of anesthesiology were evenly divided into experimental group (ultrasound-guided nerve stimulation) and control group ( traditional blind intubation method) according to random number table. Each doctor performed operation on five patients. Two groups of residents received 2 hours of pre-anesthesia training integrating theory teaching, multimedia-assisted teaching, simulated teaching and clinical teaching. We observed the two groups in anesthesia time in the brachial plexus block anesthesia,effect of anesthesia,complications as well as the patients' and residents v satisfaction. Results The experimental group was superior to the control group because of better anesthesia effect and significantly reduced onset time (P 〈0. 05 ) and significantly increased satisfaction from the patients and residents ( P 〈 0. 05 ). Conclusion Applying ultrasound-guided nerve stimulation in clinical teaching of the brachial plexus block can increase the success rate and satisfaction of patients and residents.
