微波消融与腹腔镜肝切除术治疗肝血管瘤的临床疗效及对患者肝功能的影响对比

摘　　要：目的分析肝血管瘤患者应用微波消融与腹腔镜肝切除术的临床治疗效果及对肝功能的影响。方法选取南京医科大学附属南京医院2012年7月-2014年7月收治的60例肝血管瘤患者,运用双色球随机分组法分为甲组(n=30)及乙组(n=30),甲组患者采用微波消融治疗,乙组患者接受腹腔镜肝切除术治疗,评估两组患者临床治疗效果及对肝功能的影响。结果两组患者手术时间、出血量比较,差异有统计学意义(P<0.05);并发症发生率、住院时间比较,差异无统计学意义(P>0.05);甲组患者术后12、24、48 h VAS评分均低于乙组,差异有统计学意义(P<0.05);术后1 d两组患者肝功能指标差异有统计学意义(P<0.05);术后3 d差异无统计学意义(P>0.05);随访两组患者均恢复良好。结论在肝血管瘤治疗过程中,微波消融与腹腔镜肝切除术疗效相当,均为微创、安全有效的治疗方法,临床应综合考虑患者病情、体质等,合理选择手术方法,提高手术治疗效果,具有重要的应用价值。Objective To analyze the clinical therapeutic effect of microwave ablation and laparoscopic hepatectomy in patients with hepatic hemangioma,and investigate the effect of microwave ablation on liver function. Methods Sixty cases of hepatic hemangioma patients were selected from Jul. 2012 to Jul. 2014,all patients were divided into group A（n = 30） and group B（n = 30）,patients in group A received microwave ablation firstly,patients in group B received laparoscopic hepatectomy. Clinical therapeutic effect and influence on liver function were compared between two groups.Results There were significant differences in operation time and amount of bleeding between two groups（P〈0. 05）;complication rate,hospitalization time had no significant difference between two groups（P〉0. 05）; after 12,24 and48 hours treatment,VAS score was lower in group A than that in group B（P〈0. 05）; after 1 day operation,liver function indexes had significant difference between two groups（P〈0. 05）; after 3 days operation,there was no significant difference（P〉0. 05）. Followed-up of two groups,patients recovered well. Conclusion In the treatment of hepatic hemangioma,the treatment effect of microwave ablation and laparoscopic hepatectomy are quite,they are minimally invasive,safe and effective treatment methods.
