摘　　要：目的分析急性胰腺炎(acute pancreatitis,AP)发病早期CD3^-CD16^+CD56^+NK细胞计数在AP严重程度预测中的作用。方法采用流式细胞仪荧光抗体标记法分别对164例AP患者的外周血单核细胞进行CD3、CD16、CD56荧光抗体标记,分析比较AP患者CD3^-CD16^+CD56^+NK细胞在外周血单核细胞中的百分比和计数情况,及其与AP严重程度的关系。用ROC曲线和AUC评价CD3^-CD16^+CD56^+NK细胞在外周血单核细胞中的百分比和计数对SAP的诊断价值。结果与正常对照组相比,MAP组、MSAP组和SAP组的CD3^-CD16^+CD56^+百分比和计数均显著下降,各组间差异有统计学意义(P<0.05)。CD3^-CD16^+CD56^+细胞百分比的AUC为0.86(95%CI:0.745~0.973),取值≤5.27%时,特异性为0.92,敏感性为0.79;CD3^-CD16^+CD56^+细胞计数的AUC为0.89(95%CI:0.808~0.961),取值≤0.056×109/L时特异性为0.90,敏感性为0.79。结论 CD3^-CD16^+CD56^+NK细胞在外周血单核细胞中的百分比和细胞计数对SAP早期诊断有一定的价值。Objective To analyze the CD3^-CD16^＋CD56^＋NK cell counts of patients in the early stage of acute pancreatitis（AP）,and analyze its role in the prediction of AP severity. Methods All of 164 AP patients were enrolled in the study. By labeling CD3,CD56,CD16 fluorescent antibodies,the percentage and counts of CD3^-CD16^＋CD56^＋NK cell in peripheral blood mononuclear cell were analyzed by flow cytometry. The relationship between CD3^-CD16^＋CD56^＋NK cell percentage and counts and the severity of AP was analyzed. ROC curves and AUC were used to evaluate the diagnostic value of CD3^-CD16^＋CD56^＋NK cells in severe AP（SAP）. Results Compared with normal control group,CD3^-CD16^＋CD56^＋NK cell percentage and counts were significantly reduced in AP patients. The obvious differences were also found among MAP group,MSAP group and SAP group（P 0. 05）. CD3^-CD16^＋CD56^＋NK cell percentage of the AUC was 0. 86（95% CI：0. 745 - 0. 973）. When the value was less than or equal to 5. 27%,specificity was 0. 92,sensitivity was 0. 79. CD3^-CD16^＋CD56^＋NK cells count of the AUC was 0. 89（95% CI：0. 808 - 0. 961）.When the value was less than or equal to 0. 056 × 109/L,specificity was 0. 90,sensitivity was 0. 79. Conclusion The percentage and counts of CD3^-CD16^＋CD56^＋NK cell in peripheral blood mononuclear cells may have a diagnostic value for early diagnosis of SAP.
