机器人辅助腹腔镜肾部分治疗老年T1a期肾癌15例报道兼文献复习

摘　　要：目的总结机器人辅助腹腔镜保留肾单位肾部分切除术在老年患者中应用的经验,探讨此术式在老年患者中的疗效及安全性。方法回顾分析15例老年病例临床资料,患者接受机器人辅助腹腔镜下肾部分切除术。结果 15例患者手术均顺利进行,无中转开放手术者。手术时间(不包括机器人准备时间)(165±10.5)min;术中失血量(125.0±8.2)mL,无术中输血;热缺血时间(23.5±5.8)min;无术中并发症。术后住院时间平均9.5 d。术后病理报告肿瘤切缘均为阴性。随访1-35个月(平均20.2月),无复发和转移。结论 :机器人辅助腹腔镜保留肾单位肾部分切除术对高龄局限性小肾癌患者是安全有效的。在肾肿瘤的完整切除及肾脏创面的缝合上有明显的优势。Objective To discuss the application of robotic-assisted laparoscopic partial nephrectomy （RALPN） in the elderly patients. Methods A retrospective analysis was made to the clinical data of 15 elderly patients receiving RALPN. Results All the operations were successful with no conversion to open operation; the operation time was （165 ± 10.5） minutes, the bleeding volume in operation was （125.0±8.2） ml, no blood transfusion was made in operation; the warm ischemic time was （23.5 ±5.8） minutes; no complications occurred in operation; the average time of hospitalization was 9.5 days; the histological results showed that the tumors were totally removed and the surgical margin was negative; followup lasted from 1 month to 35 months with the average of 20.2 months, and no reoccurrence and metastasis were found. Conclusions RAPN is safe and effective in elderly patients with early renal carcinoma, and it has the obvious advantage in complete excision of renal carcinoma and in the suture of kidney incision.
