老年患者拔牙前焦虑特征和主要影响因素的分析

摘　　要：目的探讨老年患者拔牙手术前的焦虑特征和术前焦虑的主要相关影响因素,旨在明确缓解拔牙手术焦虑的有效方法。方法选择30例拔牙手术老年患者为老年组(观察组),30例拔牙手术非老年患者为非老年组(对照组)。运用改良牙科焦虑量表对两组患者进行焦虑程度评分,分析两组患者焦虑相关因素,对两组差异进行统计学分析。结果拔牙前与非老年组比较,老年组重度焦虑的百分率显著升高(P<0.01),轻度焦虑的百分率明显降低(P<0.01),中度焦虑百分率比虽降低,但无统计学意义。与非老年组比较,老年组进行拔牙、用探针检查牙龈或龋洞和听到牙钻声音的发生率均明显升高(P<0.01、P<0.05)。结论拔牙术前老年患者的焦虑程度显著高于非老年患者;老年拔牙焦虑的相关影响因素主要有拔牙、用探针检查牙龈或龋洞和听到牙钻的声音等。拔牙前应当充分重视老年患者的焦虑及其相关影响因素,并需要及时干预。Objective To discuss the anxiety characteristics and anxiety-related factors in elderly patients before tooth extraction so as to find out an effective way to relieve anxiety. Methods 30 elderly patients were selected to be observation group while 30 non-elderly patients were selected to be control group; the modified dental anxiety scale was applied in assessing the anxiety degree of the cases in both groups and the anxiety-related factors were analyzed; statistical study was made to the difference between the 2 groups. Results Before tooth extraction, the percentage of severe anxiety in observation group increased much more than that in control group （P 〈 0.01） while the percentage of mild anxiety in observation group decreased much more than that in control group （P 〈 0.01）; the percentage of moderate anxiety in observation group decreased more than that in control group with no difference of statistical significance; tooth extraction performance, the gum or cavity probing and the dental drilling sound increased the anxiety of the patients in observation group much more than in control group （P 〈 0.01, P 〈 0.05）. Conclusions The anxiety degree of the patients in observation group before tooth extraction is higher than that in control group; tooth extraction performance, the gum or cavity probing and the dental drilling sound are the anxiety-related factors, which need a timely intervention.
