摘  要:目的探讨IgG4相关疾病(IgG4-related disease,IgG4-RD)与人类白细胞抗原(human leukocyte antigen,HLA)-DR、DQ等位基因的相关性。方法共纳入北京协和医院IgG4-RD队列研究患者91例,健康对照100例,采用序列特异性引物-聚合酶链反应(polymerase chain reaction-sequence specific primer,PCR-SSP)检测方法进行HLA-DR、HLA-DQ基因亚型分型。结果与健康对照组相比,IgG4-RD患者HLA-DRB1*03(13%vs.5%,P=0.047)表达率显著增高;HLA-DRB1*09表达率显著降低(16%vs.33%,P=0.009)。根据脏器受累不同,其基因表达率亦不相同。其中HLA-DRB1*03、HLA-DRB1*08在胰腺受累患者,HLA-DRB1*07、HLA-DQB1*02在颌下腺受累患者,HLA-DRB1*03、HLA-DQB1*02在泪腺受累患者,HLA-DRB1*04、HLA-DRB1*07、HLA-DQB1*02在腮腺受累患者,HLADRB1*07在泌尿系统受累患者,HLA-DRB1*03、HLA-DRB1*07、HLA-DQB1*02在鼻窦受累患者,HLA-DRB1*08在腹膜后受累患者,HLA-DRB1*04在肺部受累患者表达率显著升高。结论 IgG4-RD患者携带HLA-DRB1*03频率较高,该基因可能与疾病易感有关。不同脏器受累患者HLA-DR、HLA-DQ的基因亚型表达率不同。Objective To analyze the correlation between HLA-A DR, DQ alleles and IgG4-related diseases (IgG4-RD). Methods HLA-DR, DQ alleles were tested in 91 patients with IgG4-RD and 100 normal controls by polymerase chain reaction-sequence specific primers (PCR-SSP). Results The frequency of HLA-DRB1 * 03 in IgG4-RD group was much higher than that in control group with significant statistical difference ( 13% vs. 5% , P =0. 047) ; The frequency of HLA-DRB1 * 09 in IgG4-RD group was much lower than that in control group with significant statistical difference ( 16% vs. 33%, P = 0. 009 ). In addition, the gene expression level also showed difference according to different organ involvement. The frequency of HLA-DRB1 * 03 and HLA-DRB1 * 08, HLA-DRBI * 07 and HLA-DQB1 * 02, I-ILA-DRB1 * 03 and HLA-DQB1 * 02, HLA-DRB1 * 04, HLA-DRB1 * 07 and HLA-DQB1 * 02, HLA-DRB1 * 07HLA-DRB1 * 03, HLA-DRB1 * 07 and HLA-DQB1 * 02, HLA-DRB1 * 08, as well as HLA-DRB1 * 04 were higher in patients with of pancreatic gland, submandibular gland, lacriminal gland, parotid, urinary tract, sinus, retroperitoneal fibrosis and lung involvement, respectively. Conclusion Our results suggest that HLA-DRB1 * 03 might be the susceptible gene in IgG4-RD. The gene expression also shows differ- ence according to different organ involvement.
