四种方案治疗青少年抑郁症伴精神病性症状患者的疗效与经济学研究

摘　　要：目的:探讨治疗伴精神病性症状的青少年抑郁症患者的最佳药物治疗方案。方法:选取广东省人民医院精神卫生中心收治的青少年抑郁症患者作为研究对象,进行回顾性研究。筛选出4组不同的用于治疗青少年抑郁症的药物治疗方案,A组患者(10例)采用富马酸喹硫平片+盐酸度洛西汀肠溶胶囊治疗,B组患者(12例)采用盐酸文拉法辛缓释胶囊+利培酮片治疗,C组患者(10例)采用奥氮平片+盐酸度洛西汀肠溶胶囊治疗,D组患者(10例)采用富马酸喹硫平片+盐酸舍曲林片治疗。观察4组患者的疗效、不良反应,并进行成本-效果分析、敏感度分析。结果:A、B、C、D组患者治疗总有效率分别为80%(8/10)、50%(5/12)、60%(6/10)、100%(10/10),D组明显高于其他组,差异有统计学意义(P<0.05);4组患者不良反应发生情况的差异无统计学意义(P<0.05)。4组药物治疗方案中,D组的成本最小,效果最好,成本-效果比最小;敏感度分析结果显示,D组的成本效果比最低。结论:富马酸喹硫平片+盐酸舍曲林片治疗青少年抑郁症,成本-效果最优,安全性好,是本研究中的最优方案。OBJECTIVE： To probe into the optimized drug therapies in treatment of adolescent depression patients with psychiatric symptoms. METHODS： Adolescent depression patients with psychiatric symptoms admitted into Mental Health Center of Guangdong Academy of Medical Sciences were selected as the research objects,and retrospective analysis was conducted.Four kinds of therapies on adolescent depression patients with psychiatric symptoms were screened. Patients in group A（ 10 cases） were given Quetiaping tablets and Duloxetine enteric capsules,group B（ 12 cases） were given Venlafaxind sustained-release capsules and Risperidone tablets,group C（ 10 cases） were treated with Olanzapine and Duloxetine enteric capsules,and group D（ 10 cases） received Quetiaping tablets and Sertraline tablets. The efficacy,adverse drug reactions of four groups were observed. Cost-effectiveness analysis and sensitivity analysis were conducted. RESULTS： The total effective rate of group A,B,C and D were respectively 80%（ 8 /10）,50%（ 5 /12）,60%（ 6 /10） and 100%（ 10 /10）,group D was significantly higher than other groups,with statistically significant difference（ P〈0. 05）. There was no statistical significance among four groups in the incidence of adverse drug reactions. Of the four kinds of therapies,the cost of group D was the lowest while the effects was the best,with the minimum C / E. According to the sensitivity analysis,group D had the best cost-effect ratio. CONCLUSIONS： Quetiaping tablets and Sertraline tablets in treatment adolescent depression patients with psychiatric symptoms has the best cost-effect ratio with high safety,which the optimized therapies of all.
