摘 要:目的通过了解HBs Ag阳性孕产妇血清中HBV DNA含量和宫内感染率的关系,探讨不同护理方式对HBs Ag阳性孕产妇的产程护理与新生儿HBV的阳性率的影响以及孕产妇护理满意度的情况。方法应用荧光定量PCR法检测孕产妇及新生儿血清HBV DNA含量,酶联免疫(ELISA)法检测孕妇乙肝五项和新生儿HBs Ag;观察不同的护理方式对新生儿HBV阳性率的影响。结果被调查新生儿宫内感染率为9.28%。其中母亲HBV DNA≥107copy/ml和<107copy/ml的HBV宫内感染率分别为38.14%、2.64%,两者差异有统计学意义(P<0.05);母亲HBe Ag阳性和阴性的HBV宫内感染率分别为38.46%和2.44%,两者差异有统计学意义(P<0.05)。观察组孕产妇护理满意度为97.9%,明显高于对照组(81.3%),差异有统计学意义(P<0.05)。结论孕妇血清HBV DNA含量和HBe Ag水平与新生儿宫内感染相关。对HBV携带者孕产妇治疗的同时,加强产程观察并做好护理工作不仅可以降低新生儿感染率,还可以提高产科护理质量。Objective By understanding the relationship between HBV DNA content in serum HBs Ag positive pregnant women and intrauterine infection,to explore the effect of different ways of nursing on the care labor to HBs Ag positive pregnant women and the positive rate of newborn HBV,as well as the satisfaction of maternal care. Methods Maternal and neonatal serum HBV DNA content was detected by fluorescence quantitative PCR,and enzyme immunoassay( ELISA) was used to detect 5 items of hepatitis B in pregnant women and newborns' HBs Ag; to observe the effect of different nursing methods on the positive rate of HBV in newborn infants. Results Neonatal intrauterine infection rate was 9. 28%. Wherein the HBV intrauterine infection rates of mothers' HBV DNA≥107copy/ml and 107 copy/ml were 38. 14% and 2. 64%,and the difference was statistically significant( P 0. 05); that of mother with HBe Ag positive and negative were 38. 46% and 2. 44%,respectively,and the difference was statistically significant( P 0. 05). Care satisfaction of observation group was 97. 9%,which was significantly higher than the control group( 81. 3%),and the difference was statistically significant( P 0. 05). Conclusion Maternal serum HBV DNA levels and HBe Ag levels are related to intrauterine infection. Simultaneous treatment of maternal HBV carriers,strengthening labor observation and care work can not only reduce neonatal infection,but also improve the quality of obstetric care.
