摘 要:目的探讨超声NT测量与血清β-HCG及PAPP-A检测在孕11~13^(+6)w唐筛中的作用和意义,以期对现有研究数据进行补充,从而为临床诊疗提供指导。方法回顾性三亚市妇幼保健院2014年08月至2016年08月期间经临床确诊为唐氏综合征的孕11~13^(+6)w单活胎妊娠孕妇38例,且均予实施超声NT测量与血清β-HCG及PAPP-A检测。最终根据资料分析单一筛查与联合筛查的结果。结果经超声NT测量的阳性率为84.21%(32/38),经血清β-HCG及PAPP-A检测的阳性率为76.32%(29/38),经超声NT测量联合血清β-HCG及PAPP-A检测的阳性率为97.37%(37/38)。其中,联合筛查较单一筛查,差异均有统计学或显著性统计学意义(P<0.05或0.01)。但是,单一筛查之间比较,差异无统计学意义(P>0.05)。结论超声NT测量与血清β-HCG及PAPP-A检测在孕11~13^(+6)w唐筛中不能互相取代,两者联合筛查可提高筛查的精准性,对诊断胎儿唐氏综合征有着积极的作用和意义。Objective: To explore the functions and importance of tang sieve for ultrasonic NT measurement and serum β-HCG and PAPP-A detection in the 11 - 13 +6 weeks of pregnant women in order to supplement existing research data so that to provide guidance for clinical diagnosis and treatment. Methods: Retrospectively during August 2014 to August 2016 by the clinical diagnosis of down syndrome 11 - 13+6 weeks of single live pregnancy pregnant women 38 cases, who were come from Maternity and Child Health Hospital of Sanya. And to implement the ultrasonic NT measurement and serum β-HCG and PAPP-A detection. According to data analysis the results of a single screening and combined screening. Results: The positive rate was 84.21% (32/38) of ultrasonic NT measurement, the β-HCG and PAPP-A detection of serum positive rate was 76.32% (29/38) and by ultrasound NT measuring combined ^-HCG and PAPP-A detection of serum positive rate was 97.37% (37/38) . Among them, the comparation of a single screening, differences have statistical or significant statistical significance (P〈0.05 or 0.05) . However, comparison between a single screening, there was no statistically significant difference (P〉0.05) . Conclusion: The tang sieve for ultrasonic NT measurement and serum β-HCG and PAPP-A detection in the 11- 13 +6 weeks of pregnant women can not replace each other. The combination of both can improve the precision. For diagnosing fetal down syndrome have positive function and meaning.
