抗原致敏DC联合CIK治疗对肾癌术后患者免疫功能的影响

摘　　要：目的研究抗原致敏DC联合CIK治疗对肾癌术后患者免疫功能的影响。方法选取2012年3月至2014年4月苏州大学附属第二医院收治的42例肾癌术后患者(Ⅰ~Ⅲ期),男25例,女17例。中位年龄57(28~73)岁。应用免疫荧光法测定患者接受联合生物治疗前1周和治疗后1周的细胞免疫功能,并比较其变化。治疗后1周进行迟发型过敏反应(DTH)试验,观察阳性率。结果应用联合生物治疗1周后,患者的CD3^+、CD3^+CD4^+、CD3^+CD56^+、CD4^+/CD8^+比值升高,与治疗前比较差异有统计学意义(P﹤0.05);外周血中IL-2、IL-12、IFN-γ水平显著升高(P﹤0.05),机体对肿瘤抗原的特异性DTH反应明显增强,迟发型过敏反应(DTH)阳性率达73.80%(31/42),无严重不良反应。结论肾癌术后应用抗原致敏DC^+CIK联合治疗能有效提高患者的细胞免疫功能,改善免疫应答,可提高患者的生存获益,是安全有效的。Objective： To study the effect of immune function of tumor antigen-loadedsv（ DCs） in combination with cytokine-induced killer（ CIK） cells treatment on postoperative renal cancer patients. Methods： Frty-two patients with renal cell carcinoma were chosen after operation（ 25 males and 17 females,stageⅠ-Ⅲ,median age 57 years（ 28 ^ 73 years）during Mar 2012 to 2014 from The Second Affiliated Hospital of Suzhou University. The function of cell immunity in blood was measured and compared by immunofluorescence at 1 week before and after treatment respectively. The delayed type hypersensitivity（ DTH） test was made at 1 week before and after treatment and the rate of DTHwas observed. Results： A week after treatment,the percentages of CD3^＋,CD4^＋,CD3^＋CD56^＋and the ratio of CD4^＋/CD8^＋increased significantly compared to no-treatment（ P 0. 05）,the levels of IL-2,interferon gamma（ IFN-γ） increased significantly（ P 0. 05）,Specific DTH reactions to a tumor antigen were obviously enhanced. The positive rate of DTH was 73. 80%（ 31/42）,there was no serious adverse reaction. Conclusion： Treatment with tumor cell antigen-loaded cells in combination with CIK cells is safe and effective for postoperative renal cancer patients. The function of cell immunity is enhanced,and the immunological response of the tumor cellsi improved. It can improve the survival benefit of patients＇.
