摘 要:目的:研究携有胰岛素样生长因子-1(IGF-1)基因的重组单纯疱疹病毒-1(HSV-1)载体对糖尿病大鼠勃起功能障碍(erectile dysfunction,ED)病变的治疗作用。方法:选用8周龄雄性Sprague-Dawley(SD)大鼠,应用链脲佐菌素(Streptozotocin,STZ)诱导糖尿病8周,阿朴吗啡(apomorphine,APO,80 ug/kg)皮下注射进行阴茎勃起试验,筛选出糖尿病ED大鼠模型。随机选取SPF级雄性SD大鼠10只为CN组,从筛选出DMED大鼠模型中随机选取40只,分为DM组,IGF-1组,INS组,IGF-1+INS组,每组10只。DM组、IGF-1组、INS组、IGF-1+INS组分别或同时每只给予胰岛素6 U/d和(或)5×10^8 pfu单纯HSV-1-IGF-1载体。基因治疗21天后,对各组大鼠的阴茎海绵体内压(ICP)进行测定。结果:电刺激诱导ICP值在CN组为(43.40±3.26)cm H2O,DM组为(13.46±1.70)cm H2O,IGF-1组(25.34±1.93)cm H2O,INS组(28.79±2.01)cm H2O,IGF-1+INS组(42.21±2.13)cm H2O。各组的IGF-1阳性细胞百分比分别为CN组为(68.24±7.16),DM组为(55.74±5.02),IGF-1组(71.08±7.08),INS组(70.35±3.89),IGF-1+INS组(72.90±2.64)。结论:糖尿病大鼠ED的发生与发展可能与大鼠阴茎中IGF-1表达水平降低有关,通过引入可表达的IGF-1基因和INS治疗可在一定程度上改善大鼠阴茎勃起功能。Objective: To study the curative treatment role of the recombinant herpes simplex virus-1 vector with insulin-like growth factor-1 (IGF-1)gene in diabetes mellitus rats with erectile dysfunction. Methods: The study selected eight weeks of male Sprague-Dawley rats and used the Streptozotocin to induce diabetes mellitus in eight weeks. The apomorphine was used to conduct a telotism experiment and screened the ED rats. We random choiced the forty diabetes mellitus rats with erectile dysfunction and the ten health rats. This fifty rats will be divided into five groups:normal control (CN group), diabetes model (DM group), HSV-I-IGF-I(IGF-1 group), insulin therapy model(INS group), HSV-I-IGF-1 & insulin therapy model group(IGF-1+INS group). IGF-1 group, INS group and IGF-I+INS group will be injected the insulin 6 IU/d and/or 5× 10^8 pfu herpes simplex virus-1 vector either or both. The intracavemous pressure of rats was analyzed to evaluate the erection function aRer gene therapy. Results: The electrical-stimulation-induced ICP values in CN group, DM group, IGF-1 group, INS group, IGF-I+INS group were 43.40± 3.26 cm H20, 13.46± 1.70 cm H20, 25.34± 1.93 cm H20, 28.79± 2.01 cm 1-120, 42.21± 2.13 cm H20. The IGF-1 positive cell percentage in priapus tissue was 68.24±7.16, 55.74± 5.02, 71.08± 7.08, 70.35± 3.89 in CN group, DM group, IGF-1 group, INS group, IGF-I+INS group, respectively. Conclusion: The decreased IGF-1 expression levels in priapus tissue may be involved in the development of diabetes mellitus rats with erectile dysfunction. With the introduction of HSV-1-IGF-1 and the insulin therapy could improved the penile erectile erection in rats.
