摘　　要：目的探讨外周血中性粒细胞与淋巴细胞比值(NLR)评估老年结肠癌患者预后的意义。方法老年结肠癌患者105例,术前检测并计算外周血NLR。绘制ROC曲线,取NLR最佳截断值3.2,将患者分为高NLR组(NLR≥3.2,45例)和低NLR组(NLR<3.2,60例),分析术前NLR与患者预后之间的关系,比较两组临床病理指标的差异。结果高NLR组患者的肿瘤大小、浸润深度、远处肝转移、改良Duke分期和淋巴结转移指标均较低NLR组差(P<0.05)。高NLR组的5年生存率低于低NLR组(20%vs.61%)(P<0.05)。NLR≥3.2为老年结肠癌患者的独立预后因素(P<0.05)。取NLR最佳截断值3.2,其预测生存状况的灵敏度为61.0%,特异度为80.4%。结论术前NLR与老年结肠癌手术患者的预后密切相关。objective To investigate the significance of the ratio of neutrophil to lymphocyte （NLR） of peripheral blood in evaluating the prognosis of elderly patients with colon cancer. Methods The NLR was calculated in 105 elderly patients with colon cancer before surgery. The receiver- operating characteristic（ROC） curve of NLR and the prognosis was drawn. On the basis of ROC curve analysis, the patients were assigned into two groups of high NLR（NLR≥3.2,45 cases） and low NLR （NLR〈3.2, 60 cases）. The relationship of NLR before surgery and the prognosis after radical operation was analyzed and the differences in clinicopathological indicators were compared between two groups. Results The clinicopathological indicators including tumor size, depth of invasion, liver metastasis,modified Duke staging, and lymph node metastasis were better in low NLR group than those in high NLR group（P〈0. 05）. The 5-year survival rate was lower in high NLR group than that in low NLR group（20% vs. 61%）（P〈0. 05）. The NLR≥3.2 was an independent prognostic factor of elderly patients with colon cancer. Taking 3.2 as the best cutoff value of NLR, the sensitivity and specificity for evaluating the prognosis of elderly patients with colon cancer after operation were 61.0% and 80.40%, respectively. Conclusion Preoperative NLR is closely related with the prognosis of elderly patients with colon cancer after operation.
