早期康复训练对急性老年脑梗死患者神经功能康复及血清IMA和NSE水平的影响

摘　　要：目的观察早期康复训练对急性老年脑梗死患者神经功能康复及血清缺血修饰清蛋白(IMA)和神经元特异性烯醇化酶(NSE)水平的影响。方法将60例急性老年脑梗死患者随机分为对照组(n=30)和康复组(n=30),对照组给予常规药物治疗,康复组在此基础上再进行早期康复训练;比较两组患者的美国国立卫生院卒中量表(NIHSS)评分、Barthal指数(BI)评分及Fugl-Meyer运动功能(FMA)评分。分别于治疗前、治疗4w后检测两组患者血清IMA及NSE水平。结果治疗4w后,两组NIHSS评分较治疗前明显降低(P<0.01),血清IMA、NSE水平均较治疗前明显降低(P<0.01),且治疗后康复组NIHSS评分明显低于对照组(P<0.05),血清IMA、NSE水平明显低于对照组(P<0.01);治疗后两组BI及FMA评分均较治疗前明显提高(P<0.01),且康复组评分明显更高于对照组(P<0.05)。结论早期康复训练有效的降低血清NSE、IMA表达水平,促进了神经功能恢复,提高了患者的生活质量。Objective To investigate the effects of early rehabilitation on neurologic function recovery and serum isehemic modified albumin （ IMA ） and neuron specific enolase （ NSE ） levels in elderly patients with acute cerebral infarction （ ACI ） . Methods 60 elderly patients with ACI were randomly divided into rehabilitation group （ n=30 ） and control group （ n--30 ） . The control group accepted routine medication, which the rehabilitation group accepted early rehabilitation trainning in addition. NIHSS score,Barthel index score and FMA score were compared between the two groups. The expression of serum IMA and NSE in two group were detected before and after 4 weeks of treatment. Results After 4 weeks, NIHSS score in two groups was lower than those before treatment （ P〈0.01 ） , serum IMA and NSE levels in both groups were lower than pretreatment （ P〈0.01 ） . NIHSS score was significantly lower than those in control group （ P〈0.05 ） , serum IMA and NSE levels in rehabilitation group were significantly lower than those in control group （ P〈0.01 ） . FMA score and BI score in both groups were significantly higher than pretreatment（ P〈0.01 ）, and those in rehabilitation group were significantly higher than those in control group（ P〈0.05 ）. Conclusion Early rehabilitation training can reduce the expression of serum IMA and NSE, promote neurologic function rehabilitation and improve the quality of life ACI patients.
