帕金森病患者认知功能和平衡障碍关系的研究

摘　　要：目的探讨帕金森病(PD)患者认知功能与平衡障碍之间的关系。方法选取本院2014年1月至2015年11月门诊及住院诊断明确的PD患者50例,通过剑桥老年人精神疾病检查量表测定认知功能的测定,,用日本公司生产的重心平衡检查系统测试平衡障碍的定量,综合分析数据,把认知功能分为轻、中、重度三种,对比于平衡障碍的定量测定数据以探究两者之间的相关性。结果正常人及PD各认知障碍程度组别患者重心摆动各参数比较结果显示,PD各认知严重程度组重心摆动面积随认知严重程度越重则增长,重心摆动面积与认知功能有较好的曲线关系,重心摆动面积增大,单位时间轨迹长增大,两组比较差异有统计学意义(P<0.05);重度患者感觉整合测试平均综合<中度<重度,三组间比较差异有统计学意义(P<0.05),经感受器分析,重度、中度、轻度患者视觉、前庭觉的平衡比率均未达正常值,与对照组比较明显降低(P<0.05)。结论随着认知功能障碍增加其平衡障碍也影响增加,PD患者认知功能障碍严重程度与平衡障碍之间存在一定的相关性。Objective To investigate the relationship between cognitive function in patients with Parkinson balance between the barriers. Method Hospital from January 2013 to November 2014 to select the source of outpatient and inpatient diagnosed 50 cases of ParHnson＇s disease （ PD ） , cognitive function was measured by the Cambridge Mental Disorders Checklist determination of the elderly, balance disorders Quantitative determination of the detector used for the Japanese company＇s center of gravity balance check system testing, a comprehensive analysis of the data, the cognitive functions are divided into mild, moderate and severe three, quantitative comparison of the measured data to balance disorders to explore between the two Correlation. Results The focus of normal subjects and patients with PD each severity group cognitive swing parameters between the results of the show, each of the severity of PD awareness group postural sway area with more severe cognitive severity of the growth area of postural sway better cognitive function curvilinear relationship, postural sway area increases, increasing the unit long time trace, the difference was significant （ P〈0.05 ） ; sensorimotor integration of patients with severe test average composite〈moderate〈severe, tris the difference between groups was statistically significant （ P〈0.05 ） , by receptor analysis, severe, moderate and mild in patients with visual, vestibular sense of balance did not reach the normal rate, and the control group was significantly lower （ P〈0.05 ） . Conclusion With the cognitive dysfunction increase its balance disorder also affects the increased cognitive dysfunction in patients with PD there is a certain correlation between the severity and balance disorders.
