摘  要:目的探讨微小染色体维持蛋白-5(MCM5)与P16^(INK4A)在HPV16感染的宫颈鳞癌中表达及意义。方法采取实时荧光定量PCR及免疫组化,测定存在HPV16感染的正常宫颈、CIN 1、CIN 2/3及宫颈鳞癌组织中MCM5、P16^(INK4A)的mRNA、蛋白表达情况。结果 (1)MCM5 mRNA及蛋白在HPV16感染的不同程度宫颈病变组织中表达趋势一致,随CIN程度的增加表达增高。MCM5 mRNA及蛋白在鳞癌中表达显著较各级别上皮内病变及正常组织高(P<0.01)。MCM5蛋白的表达和年龄无关联(P>0.05);与病理分级、临床分期相关(P<0.01);(2)不同程度宫颈病变中P16^(INK4A)mRNA、蛋白表达趋势一致,随CIN程度增加表达增高。P16^(INK4A) mRNA及蛋白在鳞癌中的表达明显高于正常及CIN 1组织(P<0.01);和CIN2/3之间差异无统计学意义(P>0.05)。P16^(INK4A)蛋白在宫颈癌中的表达和患者年龄及临床分期没有关联(P>0.05);和病理分级有关(P<0.01);(3)MCM5和P16^(INK4A)蛋白表达呈正相关(r=0.497,P<0.01)。结论宫颈鳞癌中MCM5和P16^(INK4A)存在高表达,可能与其发生、进展相关。MCM5可较好的反映宫颈的恶性增生,与P16^(INK4A)联合检测对于完善CIN分级及预后的判断具有一定的参考价值。objective To investigate the expression and significance of minimal maintenance protein-5 and P16^INK4A in cervical carcinoma and cervical lesions with different degrees of HPV16 infection. Methods Real-time fluorescence quantitative PCR and immunohistochemistry were used to detected the expression of mRNA and protein of MCM5 and P16^INK4A in 20 cases of normal cervical tissues, 14 cases of CIN1, 18 cases of CIN2/3, 50 cases of cervical squamous cell carcinoma. Result ① The expression level of MCM5 mRNA and protein was increased with the degree of CIN expression increased. Expression of MCM5 mRNA and protein in cervical carcinoma was significantly higher than that in normal and all levels of cervical lesions (P 〈 0.01). Expression of MCM5 protein in cervical cancer was not correlated with age (P 〉 0.05), while was correlated with pathological grade and clinical stages (P 〈 0.01). ② The expression trends of P16INK4AmRNA and protein in cervical lesions in different degrees of HPV16 infection were consistent, and the expression increased with the increase of CIN degree. The expression level of P16INK4A protein in cervical carcinoma was significantly higher than that in normal and CIN1 tissues (P 〈 0.01). There was no significant difference between CIN2/3 and (P 〉 0.05). Expression of mRNA P16^INK4A and protein was not correlated with age and clinical stage (P 〉 0.05), and was correlated with pathological grading (P 〈 0.01). ③ Expression of MCM5 protein and P16^INK4A protein was positively correlated ( r=0.497, P 〈 0.01). Conclusions There is high expression of MCM5 and P16^INK4A in cervical carcinoma, which may be related to the occurrence and progress of cervical cancer. MCM5 can be a better reaction of cervical malignant hyperplasia, and P16^INK4A joint detection for the improvement of CIN classification and prognosis of the significance of the judgment.
