摘  要:目的分析华东地区宫颈非角化型鳞状细胞癌组织中人乳头状瘤病毒(HPV)感染基因型的分布及其分型数据库建立的意义。方法采用基因扩增结合基因芯片技术对506例宫颈非角化型鳞状细胞癌组织标本进行23种HPV分型检测,并对患者的相关资料进行分析。结果 506例宫颈非角化型鳞状细胞癌检出HPV感染者456例,总的HPV感染率为90.1%,其中一型感染率为75.9%(384/506),多型感染率为14.3%(72/506)。结论宫颈非角化型鳞状细胞癌组织中,HPV以一型感染为主,多型感染为辅,出现频率最多的型别依次为16、18、33、58、52、31、59、45型,是致宫颈非角化型鳞状细胞癌的主流型别,16型尤其突出,占高危型出现频率的65%(344/529)。HPV基因芯片检测技术可用于宫颈鳞状细胞癌组织的分型检测,对我国女性宫颈癌的防治、HPV疫苗的研发以及宫颈癌组织HPV感染基因型数据库的建立都具有十分重要的意义。Objective To compare the clinical distribution of 23 human papillomavirus genotypes in tissues of cervical non-keratinizing squamous cell carcinomas ( CNKSCC ) and its clinical significance of establishing of HPV genotypes database in Eastern China. Methods Polymerase chain reaction (PCR) and gene-chips technology were utilized for the detection of 23 kinds of HPV genotypes in tissue specimens from 506 cases of CNKSCC. And related materials of all subjects were analyzed. Results In 506 cases of CNKSCC, total infection rate of HPV was 90. 12% ( 456/506 ), including single genotype infection rate of HPV was 75.89% ( 384/506 ), multiple genotypes infection rate of HPV was 14. 23% ( 72/506 ). Conclusions Single genotype is a primary way of HPV infection, and multiple genotypes are secondary way of HPV infection in tissues of CNKSCC. Present frequency of top several places of HPV infection are types 16, 18, 33,58, 52, 31,59 and 45, successively; these types are main stream genotypes in tissues of CNKSCC. Type 16 is especially outstanding, and present frequency of 16 type infection is 65.03 % (344/529)in all HPV high-risk types. PCR and gene-ehip technology can detect single and multiple HPV genotypes (23 types) in cervical tissues of sqnamous cell carcinomas. Detection of HPV genotypes could be used to prevent and treat eases of HPV infection in patients with squamous cell carcinomas of the cervix and further to provide reference for the research and development of HPV vaccine that have important significance for establishing of HPV genotype database in women of squamous cell carcinomas of cervix.
