摘 要:目的本研究从蛋白水平分析Mel-18、Notch-1和PLK-1在不同阶段乳腺组织中的表达情况,探讨三者在乳腺癌发生、发展中可能的作用。方法收集正常乳腺组织、导管上皮不典型增生及乳腺癌标本共176例,采用罗氏全自动免疫组化染色法检测Mel-18、Notch-1和PLK-1在不同乳腺组织中的表达,比较其差异并分析Mel-18、Notch-1和PLK-1在不同乳腺疾病中的表达情况以及与主要病理学特征的关系。结果①Mel-18在正常乳腺组织中的表达明显高于乳腺癌,差异显著(P<0.05);Notch-1和PLK-1在乳腺癌中的表达要明显高于正常乳腺组织(P<0.05)。②Notch-1的过表达与组织学分级、临床分期及淋巴结转移呈正相关(P<0.05),与年龄和肿瘤直径之间无明显相关性(P>0.05)。PLK-1的表达仅与肿瘤的组织分级呈正相关(P<0.05),与患者年龄、肿瘤直径、临床分期及淋巴结转移之间无明显相关性(P>0.05)。结论 Mel-18为乳腺癌的肿瘤抑制基因,由于乳腺癌中Notch-1通路的激活使得乳腺增殖加速,细胞的分化状态降低。PLK-1蛋白的过量表达引起乳腺细胞的分裂和增殖,促进了乳腺癌的形成。联合检测3个指标对判定乳腺癌的预后有重要意义。Purpose To study the expression of Mel-18, NOTCH1 and PLK-1 at protein level in different stages of breast tissues and to explore the possible role of the three markers in the occurrence and development of breast cancer. Methods The normal breast, atypical ductal hyperplasia and breast cancer specimens (176 cases) were collected; using the Roche automated immunohistochemical staining we detected the expression of Mel-18, Notch-1, PLK-1 in different breast tissues. The relationship between the expression of Mel-18, Notchl, and PLK-1 and pathological characteristics of different breast diseases was analyzed. Results The expression of Mel-18 in normal breast tissue was significantly higher than that in breast cancer (P 〈0. 05) ; the expression of Notch-1 and PLK-1 in breast cancer was higher than that in normal breast tissue ( P 〈 0. 05 ). The expression of Notch-1 and histological grade, clinical stage and lymph node metastasis were positively correlated ( P 〈 O. 05 ), but no correlation with age and diameter of tumor ( P 〉 0.05 ). The expression of PLKI was positively correlated with tumor grade ( P 〈 0. 05 ), but not with the age of patients, tumor size, clinical stage and lymph node metastasis (P 〈 0. 05). Conclusion Mel-18 is a tumor suppressor gene in breast cancer, due to the activation of Notchl pathway in breast cancer makes breast proliferation and reduces cell differentiation state. The overexpression of PLK1 protein induces the mitosis and proliferation of breast cells, and promotes the formation of breast cancer. The combined detection of these three indicators plays an important role in evaluation of prognosis in patients with breast cancer.
