食管癌患者治疗前后外周血淋巴细胞亚群变化的分析

摘　　要：目的探讨食管癌患者治疗前后外周血淋巴细胞亚群的变化及临床意义。方法选取2012年1月~2014年12月在我院治疗的47例食管癌患者的临床资料进行回顾性分析,检测患者治疗前后的外周血淋巴细胞亚群,分析不同分期、是否有淋巴结转移、随访是否发生复发或转移患者外周血淋巴细胞亚群的水平。结果治疗后的CD3+、CD4+水平高于治疗前,CD8+、CD56+水平低于治疗后,差异有统计学意义(P<0.05)。Ⅰ~Ⅱ期患者治疗前的外周血淋巴细胞CD3+、CD4+水平显著高于Ⅲ~Ⅳ期,差异有统计学意义(P<0.01)。Ⅰ~Ⅱ期患者治疗后的CD4+水平显著高于Ⅲ~Ⅳ期治疗后,差异有统计学意义(P<0.01)。无淋巴结转移患者治疗前的外周血淋巴细胞CD56+水平显著低于有淋巴结转移患者,差异有统计学意义(P<0.01)。无淋巴结转移患者与有淋巴结转移患者治疗后的CD3+、CD4+、CD8+、CD56+、CD19+水平比较,差异无统计学意义(P>0.05)。治疗前及治疗后无转移或复发患者的CD4+水平显著高于发生转移及复发患者,CD8+、CD56+及CD19+水平显著低于转移或复发患者,差异有统计学意义(P<0.05)。结论食管癌患者存在明显的细胞免疫抑制情况,检测淋巴细胞亚群水平可用于监测患者免疫状态,评估患者的病情,预测患者复发及转移风险。Objective To explore the changes of peripheral blood lymphocyte subsets in patients with esophageal cancer before and after treatment.Methods The clinical data of 47 patients with esophageal cancer who were treated in our hospital from January 2012 to December 2014 were retrospectively analyzed.Lymphocyte subsets in peripheral blood of patients before and after treatment were detected.The levels of peripheral blood lymphocyte subsets in patients with dif- ferent stages,lymph node metastasis and follow-up whether recurrence or metastasis were analyzed.Results The level of CD3＋,CD4＋ after treatment was higher than that before treatment,the level of CD8＋,CD56＋ after treatment was lower than that before treatment,with significant difference （P〈0.05）.The levels of CD3＋ and CD4＋ of peripheral blood lymphoeytes in stage Ⅰ and Ⅱ before treatment was higher than that in stage Ⅲ and Ⅳ,with significant difference （P〈0.01）.The levels of CD3＋ and CD4＋ of peripheral blood lymphocytes in stage Ⅰ and Ⅱ after treatment was higher than that in stage IU and IV,with significant difference （P〈0.01）.Tbe level of CD56＋ of peripheral blood lymphocytes before treat- ment in the patients without lymph node metastasis was lower than that in the patients with lymph node metastasis,with significant difference （P〈0.01）.There was no significant difference in the level of CD3＋,CD4＋,CD8＋,CD56＋,CD19＋ of pe- ripheral blood lymphocytes after treatment between the patients without lymph node metastasis and lymph node metas- tasis （P〈0.05）.The level of CD4＋ in patients of no recurrence or metastasis before and after treatment was higher than that in patients of recurrence or metastasis,the level of CD8＋,CD56＋,CD19＋ in patients of no recurrence or metastasis before and after treatment was lower than that in patients of recurrence or metastasis,with significant difference （P〈 0.05）.Conclusion Patients with esophageal cancer have obvious cellular immune suppression.Detec
