自拟活血化瘀汤联合尿激酶半球后注射治疗外伤性玻璃体出血的临床观察

摘　　要：目的:观察自拟活血化瘀汤联合尿激酶半球后注射治疗外伤性玻璃体出血的临床疗效。方法:选择符合纳入标准的68例外伤性玻璃体出血患者作为研究对象,按随机数字表法分为观察组34例(34眼)和对照组34例(34眼),对照组患者进行尿激酶半球后注射,尿激酶10 000 U加入1 ml生理盐水稀释,隔日注射,10次为1个疗程,观察组在对照组的基础上口服自拟活血化瘀汤,10剂为1个疗程,治疗2个疗程,观察治疗前后视力变化情况,评价玻璃体混浊程度及临床治疗效果。结果:观察组和对照组治疗后视力较治疗前明显改善,差异具有统计学意义(P均<0.01),且观察组改善效果优于对照组,差异具有统计学意义(P<0.05)。观察组的临床总有效率为91.18%,明显高于对照组79.41%,差异具有统计学意义(P<0.05)。观察组和对照组治疗后玻璃体混浊程度均较治疗前明显减轻,差异具有统计学意义(P均<0.05),观察组改善程度明显优于对照组,差异具有统计学意义(P<0.05)。结论:自拟活血化瘀汤联合尿激酶半球后注射治疗外伤性玻璃体出血的临床疗效确切,能够有效提高视力,促进玻璃体混浊的吸收,优化了临床治疗效果,值得推广使用。Objective ： To observe the effect of Huoxue Huayu decoction combined with urokinase retrobulbar injection on traumatic vitreous hemorrhage. Methods： 68 patients with traumatic vitreous hemorrhage were admitted as study objects. All the patients were randomly divided into an observation group with 34 cases （34 eyes） and a control group with 34 patients （34 eyes）. The control group was given urokinase retrobulbar injection 10 000 U with adding lml NS, 10 times as a course; the observation group was given Huoxue Huayu decoction on the basis of the therapy in the control group, 10 doses as a courses. After 2 courses, the vision was observed before and after treatment. The degree of vitreous opacity and clinical efficacy were evaluated. Results ： The vision of observation group and control group after treatment was improved more significantly than that before treatment,and the difference was statistically significant （P 〈 0.01 ）. Moreover, the effect of vision improvement in the observation group was superior to that in the control group, and the difference was statistically significant （P 〈 0. 05 ）. The total effective rate of observation group was 91.18% , which was significantly higher than that of the control group with 79.41% ,and the difference was statistically significant （P 〈0.05 ）.
