调胃降逆汤联合西药治疗非糜烂性反流病肝胃不和证临床观察

摘　　要：目的:观察调胃降逆汤联合西药治疗非糜烂性反流病肝胃不和证的临床疗效。方法:将72例非糜烂性反流病肝胃不和证患者随机分为治疗组和对照组各36例。对照组采用西药治疗,治疗组在对照组治疗基础上加服调胃降逆汤,2组疗程均为6周。比较2组患者的中医症状积分、反流诊断问卷(RDQ)积分、临床疗效、安全性指标、治疗结束后4周和8周的复发情况。结果:临床疗效愈显率与总有效率对照组为60.61%与75.76%,治疗组为82.86%与94.29%,组间比较,差异均有统计学意义(P<0.05)。治疗后,2组10项中医症状积分、总积分与RDQ总积分、频率积分、程度积分均较治疗前下降,差异均有统计学意义(P<0.01);除咽部不适,治疗组其余9项症状的积分、总积分与RDQ总积分、频率积分、程度积分均低于对照组,差异均有统计学意义(P<0.05,P<0.01)。治疗结束后第4周和第8周,治疗组复发率均低于对照组,差异均有统计学意义(P<0.05)。在6周观察治疗期间,均未发现因药物原因所产生的不良反应事件。结论:采用调胃降逆汤协同西医常规治疗非糜烂性反流病肝胃不和证患者,可以较快改善患者的全身症状,提高临床疗效,降低复发率,疗效优于单纯西医治疗。Objective：To observe the clinical effect of Tiaowei Jiangni tang combined with western medicine in treating non-erosive reflux disease（NERD） patients with the syndrome of incoordination between the liver and stomach. Methods：Selected 72 patients,and divided them into the treatment group and the control group randomly. The control group was treated with western medicine,and the treatment group was given Tiaowei Jiangni tang based on the treatment of the control group. Treatment course lasted for two weeks.Compared symptom integral,integration of reflux disease questionnaire（RDQ）,clinical effect,safety index and recurrence situation after four and eight weeks of treatment. Results：The clinical cured and markedly effective rate and the total effective rate was 60.61%and 75.76% in the control group, 82.86% and 94.29% in the treatment group respectively,the difference being significant（P 〈 0.05）.After treatment,the ten symptom integrations and the total integration and integrations of RDQ,frequency and degree of the two groups were lower than those before treatment（P 〈 0.01）. Except pharyngeal trouble,the other nine integrations,total integration,integrations of RDQ,frequency and degree of the treatment groups were lower than those of the control group（P 〈 0.05,P 〈 0.01）. After four weeks and eight weeks of treatment,recurernce rate of the treatment group was lower than that of the control group（P 〈 0.05）. During the six-week observation treatment,no adverse reaction caused by drug was found. Conclusion：Tiaowei Jiangni tang combined with western medicine can improve systemic symptom,and promote the clinical effect and reduce the recurernce rate in NERD patients with the syndrome of incoordination between the liver and stomach,which is better than western medicine treatment alone.
