龙胆泻肝汤联合水飞蓟宾胶囊治疗非酒精性脂肪肝疗效观察

摘　　要：目的:观察龙胆泻肝汤联合水飞蓟宾胶囊治疗非酒精性脂肪肝的临床疗效。方法:将110例非酒精性脂肪肝肝胆实火上炎证患者随机分为对照组和治疗组各55例,在一系列改变不良生活方式的基础上,对照组给予水飞蓟宾胶囊治疗,治疗组给予龙胆泻肝汤联合水飞蓟宾胶囊治疗,观察2组的临床治疗效果,治疗前后测量患者的体质量指数、肝功能指标[谷丙转氨酶(ALT)、谷草转氨酶(AST)]和血脂指标[总胆固醇(TC)、甘油三酯(TG)],观察2组治疗后临床症状(乏力、纳差、腹胀、肝肿大)的改善情况。结果:对照组总有效率为78.18%,治疗组总有效率为92.73%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组ALT、AST和TC、TG均较治疗前下降(P<0.05),治疗组改善情况优于对照组(P<0.05);2组体质量指数均较治疗前下降(P<0.05),但组间对比,差异无统计学意义(P>0.05)。2组患者的乏力、纳差、腹胀、肝肿大均有所好转,治疗组各项症状的改善率均高于对照组,差异均有统计学意义(P<0.05)。结论:龙胆泻肝汤联合水飞蓟宾胶囊治疗非酒精性脂肪肝肝胆实火上炎证可改善患者的临床症状,恢复肝功能,调节血脂水平,是一种相对理想的治疗方法。Objective： To observe the clinical curative effect of Longdan Xiegan tang combined with silybin capsule in treating nonalcoholic fatty liver. Methods：Divide 110 cases of patients with nonalcoholic fatty liver into the control group and the treatment group randomly,55 cases in each. Based on a series of modification in unhealthy lifestyle,the control group was given silybin capsule, and the treatment group was given Longdan Xiegan tang combined with silybin capsule treatment.Observed the clinical curative effect of two groups, measured the body mass index, liver function indices, such as alanine aminotransferase（ALT）, aspartate aminotransferase（AST） and blood lipid indices [cholesterol（TC）, triglyceride（TG）, of patients before and after treatment, observed the improvement of clinical symptoms, such as hypodynamia, anorexia, abdominal distention, hepatomegaly, of two groups after treatment. Results： The total effective rate was 78.18% in the control group,and was 92.73% in the treatment group,the difference being significant（P 〈 0.05）. After treatment,ALT,AST and TC,TG in both groups were decreased（P 〈 0.05 compared with those before treatment）. The improvement in the treatment group was better than that in the control group（P〈0.05）. The body mass index of two groups were all decreased（P〈0.05） and there was no significant difference between the two groups（P 〉0.05）. The clinical symptoms of patients in two groups were both alleviated. The improvement rate of every index in the treatment group was higher than that in the control group（P 〈0.05）.Conclusion： Using Longdan Xiegan tang combined with silybin capsule in treating nonalcoholic fatty liver can alleviate the clinical symptoms of patients,help them recover liver function and improve blood lipid abnormality,which is relatively an ideal treatment.
