血栓弹力图在动脉瘤性蛛网膜下腔出血后凝血功能研究中的应用价值分析

摘　　要：目的探讨血栓弹力图（TEG）在动脉瘤性蛛网膜下腔出血（ASAH）后凝血功能评价中的应用价值。方法选取自2015年10月至2016年9月郑州大学第一附属医院神经内科及神经介入科收治的发病24h内的ASAH患者32例作为ASAH组，同期体检者20例作为对照组。记录2组成员传统凝血功能参数，应用TEG分析仪检测TEG各参数。结果2组成员传统凝血功能参数差异无统计学意义（P〉0．05）。与对照组比较，ASAH组TEG参数R值明显减小[（6．63±1．00）min vs（4．03±2．09）min]，血小板计数明显增高[（189．5±52．58）×10^9／L vs（233．9±68．4）×10^9／L]，Angle值明显增高[68．60°（63．63°，70．78°）vs 70．55°（68．83°，72．30°）]，差异均有统计学意义（P〈0．05）。结论TEG可用于评价ASAH后急性期凝血功能变化，精准地确定凝血功能异常环节,对该病急性期靶向治疗具有重要的指导价值。Objective To investigate the application value ofthromboelastography （TEG） in coagulation function after aneurysmal subarachnoid hemorrhage （ASAH）. Methods Thirty-two patients with ASAH with onset within 24 h and 20 healthy controls, recruited from our hospital from October 2015 to September 2016, were enrolled in this study. Traditional coagulation parameters were recorded. Parameters were analyzed by TEG. Results Traditional coagulation parameters between ASAH patients and controls were no statistically different （P〉0.05）. R value of TEG in the ASAH group was obviously decreased as compared with that in the controls, with significant difference （[4.03±2.09] min vs. [6.63±1.00]min, P〈0.05）. Platelet number and Angle value in ASAH group （[189.5±52.58]×10^9/L and 68.60 [63.63, 70.78]） were significantly increased as compared with those in the control group （[233.9±68.4]×10^9/L and 70.55 [68.83, 72.30], P〈0.05）. Conclusions TEG could be used to evaluate the coagulation function at acute stage after ASAH and target the abnormal link. This targeted therapy has an important value of cliuical guidance to this disease.
