摘  要:目的探讨帕金森病患者血尿酸与血浆谷胱甘肽水平与认知功能的相关性。方法选取帕金森病患者122例为帕金森病组,行蒙特利尔认知评估量表评分后分为不伴认知障碍组和认知障碍组,同时行帕金森病统一评价量表第3部分(UPDRSⅢ)评分和汉密尔顿抑郁量表(HAMD)评分。另选取同期体检者50例为对照组,帕金森病组和对照组均测定空腹静脉血尿酸及血浆谷胱甘肽值。结果帕金森病组的血尿酸及血浆谷胱甘肽水平明显低于对照组,差异均有统计学意义(t分别=8.23、23.72,P均<0.05);帕金森病伴认知障碍组的血尿酸、血浆谷胱甘肽水平低于不伴有认知障碍组,差异均有统计学意义(t分别=6.96、5.89,P均<0.05);帕金森病伴认知障碍组病程、UPDRSⅢ评分、HAMD评分均高于不伴认知障碍组,差异均有统计学意义(t分别=-3.83、-6.59、-3.99,P均<0.05);多因素logistic回归分析后显示,病程、UPDRSⅢ评分、血尿酸及血浆谷胱甘肽与患者认知障碍发生关系密切,是危险因素(OR分别=3.11、3.98、4.60、3.70,P均<0.05)。结论血尿酸及血浆谷胱甘肽水平的降低与帕金森病患者出现认知障碍有关。Objective To investigate the correlation between levels of serum uric acid(UA) and glutathione(GSH) with cognitive function in patients with parkinson’s disease(PD). Methods A total of 122 cases of PD were selected and assessed by the unified PD rating scale part Ⅲ(UPDRS Ⅲ),the hamilton depression scale(HAMD) and montreal cognitive assessment(Mo CA). According to the score of Mo CA,the PD group was divided into two subgroups of PD without cognitive impairment and PD with cognitive impairment. 50 cases of healthy people were selected as the control group. The levels of UA and GSH of all these objects were detected and compared. Results The serum levels of UA and GSH in the PD group was significantly lower than those in the control group(t=8.23,23.72,P〈0.05). The serum levels of UA and GSH was significantly lower in the PD with cognitive impairment group than those in the PD without cognitive impairment group(t =6.96,5.89,P〈0.05). The course of disease,the scores of UPDRS Ⅲ and HAMD in the PD with cognitive impairment group were higher than those in the PD without cognitive impairment group(t=-3.83,-6.59,-3.99,P〈0.05). Multiple logistic regression analysis showed that the incidence of PD complicated with cognitive impairment was correlated with the course of disease,the scores of UPDRS Ⅲ,and the serum levels of uric acid and glutathione(OR=3.11,3.98,4.60,3.70,P〈0.05).Conclusion The decreasing of levels of uric acid and glutathione are risk factors of cognitive impairment in PD patients.
