320排CT肝癌术前残肝体积评估的临床应用

摘　　要：目的探讨320排CT后处理工作站体积测量功能在肝癌术前残肝体积评估中的临床应用价值。方法对21例原发肿块型肝癌患者术前进行320排CT三期增强检查,并进行后处理重建,对肝脏动脉系统、门静脉及肝静脉系统进行血管成像,根据外科手术需求对全肝体积、肿瘤体积、预切除肝脏体积及残肝体积进行定量测量。术后采用水测量法测量切除肝脏体积,并对肝功能进行评价。结果 21例术前评估预切除肝体积与术后水测量肝体积结果无统计学差异(P>0.05)。术后肝功能受损较术前增加4例,其对应残肝体积率为38.5%-65.5%,平均为47.2%。所有患者术后随访生存均超过2个月。结论 320排CT后处理工作站体积测量功能可对活体肝脏体积作出准确术前评估,具有较大临床应用价值。Objective To explore the clinical value of the volume measurement function of post-processing workstation of 320- detector CT applied to preoperative evaluation of residual liver volume in patients with liver cancer.Method A total of 21 patients with tumor type primary liver cancer received preoperative three-phase enhanced scanning of 320-detector CT and post-processing reconstruction in order to form angiographic image of hepatic artery, portal vein and hepatic vein system, and the total liver volume, tumor volume, preoperatively resected liver volume and residual liver volume were measured quantitatively according to the surgical needs. After the operation, the volume of excised liver was measured by use of water measurement method, and the liver function was evaluated. Results Among all 21 patients, there was no significant difference between the assessed results of the preoperatively resected liver volume and the postoperative liver volume （P 〉 0.05）. After the operation, liver function damage was increased in four cases, and the corresponding rate of residual liver volume was 38.5%-65.5%, with an average of 47.2%. The postoperative follow-up visit showed that all patients＇ survival time lasted for more than two months. Conclusion The volume measurement function of post-processing workstation of 320-detector CT can be used for accurate preoperative evaluation of liver volume in vivo, so it is worthy of clinical application.
