摘  要:目的探讨HGF在HBV-GN患儿发病过程中的作用。方法采用免疫组织化学方法检测10例对照组(正常肾组织)及50例实验组(HBV-GN患儿)肾组织HGF的表达水平,结合临床指标(血白蛋白、补体C3、24h尿蛋白),分析不同临床类型、病理类型、病毒载量组HBV-GN患儿肾组织HGF表达差异及与临床指标的相关性。结果 HBV-GN组肾组织HGF蛋白表达水平明显高于正常对照组,两组间比较差异有统计学意义(P<0.05);单纯型肾病综合征组肾小球和肾小管HGF表达水平最高,与其他组比较,差异有统计学意义(P<0.05);单纯性血尿组表达水平最低,肾炎型NS和血尿、蛋白尿组和单纯性蛋白尿组表达水平居中,各组间比较差异无统计学意义;膜性肾病组肾小球和肾小管HGF表达水平高于系膜增生性肾小球肾炎组和膜增生性肾小球肾炎组,差异有统计学意义(P<0.05),系膜增生性肾小球肾炎组与膜增生性肾小球肾炎组比较,差异无统计学意义;高病毒载量组肾小球和肾小管HGF表达水平低于低病毒载量组,两组比较差异有统计学意义(P<0.05),相关性分析显示HGF表达水平与血白蛋白和血C3呈正相关,与24h尿蛋白定量呈负相关。结论 HGF可能在HBV-GN的发病过程中起着一定的保护作用。Objective To explore the role of HGF in the onset and deveplopment of HBV-GN. Methods The expression level of HGF in renal tissues of 50 patients with HBV-GN and 10 children of control group were detected by using immunohistochemistry. To analyse the difference of the expression level of HGF in renal tissues of patients with different clinical types,pathological types and level of HBV-DNA. The relationship between expression level of HGF and the concentration of 24h-urine protein excretion,the level of serum C3,the level of serum albumin were analyzed. Results Experiment group(HBV-GN group) had significantly higher HGF expression in the glomeruli and renal tubules compared to control group,The expression level of renal(glomeruli and renal tubules) HGF in membranous nephropathy group(MN) was significantly higher(P〈0.05) than that of in membrane proliferative glomerulonephritis group(MPGN) and in mesangial proliferative glomerulonephritis group(Ms PGN),but there was no statistical significance(P〈0.05) in MPGN group compared to Ms PGN group. The expression level of renal HGF in simple type nephrotic syndrome group was significantly higher(P〈0.05) than that of in nephritic type nephrotic syndrome group,hematuria and albuminuria group,Hematuria group and albuminuria group. The expression level of renal HGF in high level of HBV-DNA group was significantly Lower(P〈0.05) than that of in low level of HBV-GN group There was positive correlation between the level of HGF expression in HBV-GN groups and the level of serum C3 and serum albumin,but there was negative correlation between that of HGF expression and the concentration of 24h-urine protein excretion. Conclusions HGF probably postpones the process of the onset and development of HBV-GN.
