结直肠癌门静脉血VEGF浓度及癌组织VEGF表达与肝转移的关系

摘　　要：目的探讨结直肠癌患者门静脉血中血管内皮生长因子(VEGF)浓度及癌组织中VEGF的表达与结直肠癌肝转移的关系。方法检测164例结直肠癌患者的门静脉血VEGF浓度及癌组织中VEGF的表达情况,通过5年随访了解这些患者出现肝转移发生情况。结果门静脉血VEGF浓度≥250 g/L患者的肝转移率明显高于VEGF浓度<250 g/L的患者(<0.05);门静脉血VEGF浓度≥250 g/L而且肿瘤组织VEGF表达阳性者的肝转移率为80%;出现肝转移的患者门静脉血VEGF浓度明显高于无肝转移患者(<0.05);肝转移患者的癌组织标本中VEGF阳性表达率明显高于无肝转移患者(<0.05)。结论门静脉血VEGF高浓度及癌组织VEGF阳性表达与肝转移密切相关,门静脉血VEGF浓度≥250 g/L且癌组织VEGF表达阳性者肝转移发生率最高,可作为早期预测结直肠癌肝转移的指标。Objective To investigate the relationship between the hepatic metastasis of colorectal cancer and the con- centration of serum vascular endothelial growth factor （VEGF） in portal vein blood and the expression of carcinoma tis-sues VEGF. Methods The VEGF concentration in portal vein and the expression of VEGF in cancer tissues among 164 colorectal cancer patients were detected. The occurrence of liver metastasis in these patients was observed by 5 years follow-up. Results The patients whose portal vein blood serum VEGF concentration≥ 250μg/L had a higher hepatic metastasis rate （P 〈 0.05）. The average hepatic metastasis rate of the patients with serum VEGF ≥250μg/L and positive carcmoma tissues VEGF expression was 80%. The serum VEGF concentration of the portal vein in hepatic metastasis patients was significantly higher than that of the non-hepatic metastasis patients （P 〈 0.05）. The positive expression of cancer tissue VEGF in hepatic metastasis patients was significantly higher than that in non-hepatic metastasis patients （P 〈 0.05）. Conclusions The high concentration of the portal vein serum VEGF and the positive expression of VEGF in tumor tissue are closely correlated to hepatic metastasis of colorectal carcinoma. The concentration of portal vein serum VEGF ≥250μg/L together with the positive expression of cancer tissue VEGF can be used as an indicator to predict the hepatic metastasis of colorectal carcinoma in early period.
