摘 要:目的 综合分析血浆和肽素在进展性出血性脑损伤患者中的应用及与预后的临床关系。方法 2010年6月~2016年6月,选取在我院收治的100例进展性出血性脑损伤患者的临床资料作为观察组,再选取同期我院30例健康人群为对照组。采用ELISA法检测两组患者的血浆和肽素、C-反应蛋白、白介素-6和肿瘤坏死因子-α浓度。多因素相关分析颅脑外伤患者血浆和肽素浓度与入院时GCS评分的相关性,探讨其对脑外伤预后的早期预测作用;多因素分析颅脑外伤患者血浆和肽素浓度与死亡率的相关性。结果 (1)观察组血浆和肽素浓度别为(356.12±123.52)pmol/L,对照组血浆和肽素浓度为(85.92±31.02)pmol/L,观察组血浆和肽素浓度显著高于对照组(P〈0.05);(2)入院时GCS评分与血浆和肽素浓度呈负相关性(r=-0.521,P〈0.05);血浆和肽素浓度和GCS评分对进展性出血性脑损伤患者的预测价值比较差异有统计学意义(P〈0.05);(3)进展性出血性脑损伤组血浆和肽素显著高于非进展性出血性脑损伤组(P〈0.05)。结论 血浆和肽素在进展性出血性脑损伤患者中的应用价值较高,对预后预测较好。Objective To comprehensively analyze the application of plasma copeptin in patients with progressive hemorrhagic brain injury and its clinical relationship with prognosis. Methods 100 cases of progressive hemorrhagic brain injury patients with clinical data in our hospital from June 2010 to June 2016 were selected as the observation group,and 30 healthy people in our hospital at the same period were chosen as the control group. Plasma copeptin concentrations, C-reactive protein, interleukin-6 and tumor necrosis factor-α of two groups were measured by enzyme-linked immunosorbent assay(ELISA). The correlation between plasma copeptin concentration in patients with craniocerebral trauma and the GCS score on admission was analyzed by multivariate analysis,to explore its early predictive role in prognosis of traumatic brain injury.And the correlation between plasma copeptin concentration in patients with craniocerebral trauma and mortality was analyzed by multivariate analysis. Results The plasma copeptin concentration in the observation group was(356.12 ±123.52)pmol/L, significantly higher than that in the control group(P〈0.05), which was(85.92±31.02) pmol/L.There was a negative correlation between GCS score and plasma copeptin concentration at admission(r =-0.521, P〈0.05); and there was statistically significant difference in predictive value between plasma copeptin concentration and GCS score in patients with progressive hemorrhagic brain injury(P〈0.05). The levels of plasma copeptin in progressive hemorrhagic brain injury group were significantly higher than those in non-progressive hemorrhagic brain injury group(P〈0.05). Conclusion Plasma copeptin in patients with progressive hemorrhagic brain injury has a relatively high application value, with good prognosis.
