针刺治疗中风后抑郁症疗效观察及ERP成分P300差异性比较研究

摘　　要：目的探讨针刺治疗中风后抑郁症（PSD）的疗效及机制。方法选取2013年6月～2015年12月福建中医药大学附属康复医院收治的符合纳入标准并完成治疗患者52例。按就诊时间顺序，随机将其分为针刺组（27例）与药物组（25例），两组患者均治疗4周，治疗前后进行SDS、HAMD量表评定及ERP成分P300测定。结果治疗后两组SDS、HAMD量表评分比较，差异无统计学意义（P〉0．05）；治疗后两组ERP成分P300潜伏期缩短，差异无统计学意义（P〉0．05），针刺组ERP成分P300峰值的增高优于药物组，差异有统计学意义（P〈0．05）。结论针刺风池、太冲穴治疗PSD的疗效与药物治疗相近，针刺治疗PSD的机制有可能是通过针刺可以明显的提升PSD患者P300的峰值。Objective To discuss the clinical efficacy and the mechanism of PSD by acupuncture. Methods From June 2013 to December 2015, the affiliated rehabilitation hospital of Fujian University of traditional Chinese medicine was selected in accordance with the inclusion criteria and completed the treatment of 52 cases. According to the order of treatment time, randomly divided into acupuncture group （27 cases） and drug group （25 cases）, two groups of patients were treated for 4 weeks, before and after treatment for SDS, HAMD scale and ERP composition P300 determination. Results No significant difference in SDS and HAMD was noted between 2 groups after treatment （P〉0.05）.No significant difference the shorter latent of P300 was noted between 2 groups after treatment （P〉0.05）,but the peak value of P300 was significantly higher, in acupuncture group than Fluoxetine group respectively （P〈0.05）. Conclusion The methods were able to improve the symptoms of PSD patients, which the clinical efficacy was approximate agreement.The mechanism of PSD by acupuncture could be significantly improving the peak value of P300.
