摘 要:目的应用实时超声弹性成像技术评价不同年龄正常人肝组织硬度,并分析其影响因素,为正常人肝脏的弹性应变率建立一个初步的参考范围。方法健康志愿者138倒,按年龄分为A组(青少年组,年龄15~20岁),B组(青年组,21~40岁),C组(中年组,41~60岁)及D组(老年组,〉60岁),应用实时超声弹性成像技术分别对正常肝脏左、右叶进行肝脏弹性成像检查,获得组织弹性应变率。比较不同年龄组、不同性别弹性应变率的差别,并分析其与身高、体重、体重指数(BMI)的关系。结果分别与B组、c组、D组比较,A组肝左叶弹性应变率低,差异具有显著意义(0.85±0.29vs1.10±0.46,P〈0.05,0.85±0.29vs1.16±0.33,0.85±0.29vs1.28±0.30,P均〈0.01)。其余各组间差异无显著意义(P〉0.05)。与C组、D组比较,A组的肝右叶弹性应变率低,差异具有显著意义(0.74±0.26VS0.97±0.45,0.74±0.26vs1.04±0.25,P均〈O.05)。其余各组问差异无显著意义(P〉0.05)。肝脏的弹性应变率与身高、体重、体重指数无明显相关,不同年龄组内不同性别肝脏弹性应变率间差异无显著意义(P〉0.05)。结论实时超声弹性成像可无创性定量评价肝脏弹性。在正常人中,青少年组肝组织弹性应变率低。正常肝组织弹性应变率与身高、体重、体重指数、性别无明显关系,正常成人肝脏弹性应变范围约(4.7±L5)kPa。Objective Application of real-time ultrasound elasticity imaging evaluation of normal liver tissue hardness, different ages and analyzes the influence factors of liver for normal elastic strain rate to establish a preliminary reference range. Methods 138 cases of healthy volunteers by age were divided into group A (youth group, aged 15- 20) and group B (young, 21-40 years old), group C (middle-aged group, 41-60 years old) and group D (elderly group and〉60 years old), the application techniques of real- time ultrasound elasticity imaging normal liver right and left lobe of liver elasticity imaging, tissue elastic strain rate. Compare the differ- ence of different age groups and different gender elastic strain rate, and analyze it with height, weight, body mass index ( BMI ) ; Results Comparied with Group B, Group C and Group D, the strain ratio of left liver tissue of Group A was significantly lower than other groups, while the Group B(0. 85±0.29 vs 1.10±0.46, P〈0. 05 ; 0. 85±0. 29 vs 1.16±0. 33, 0. 85±0. 29 vs 1.28±0. 30, P ±〈0. 01 ) ; while eomparied with Group C and Group D, the strain ration of right liver tissue of Group A was significantly lower than the Group C and Group D (0. 74±0. 26 vs 0. 97±0.45, 0.74±0. 26 vs 1.04±0. 25, P均〈0. 05). However there were no significant differences in strain ration of left or right liver between other groups. There were no relationship with strain ratio of liver and height, weight and BMI. There were no significant different between strain ration of different sexes( P〉0. 05 ). Conclusions Real-time ultra- sound elastography can noninvasive quantitative evaluation of liver elasticity. In a normal person, youth group of liver tissue elastic strain rate is low. Normal liver tissue elastic strain rate and height, weight, bmi, gender has no obvious relation, normal adult liver e- lastic strain range about kPa (4. 7-1.5).
