三种角膜修复剂促角膜损伤修复的临床效果观察

摘　　要：目的:观察三种常用角膜修复剂治疗干眼症引起的角膜上皮损伤的临床效果。方法:160例(211只眼)具有角膜上皮损伤的干眼症患者随机分为四组,分别使用三种不同的角膜修复剂(重组牛碱性成纤维细胞生长因子滴眼液、小牛血去蛋白提取物眼用凝胶、0.3%玻璃酸钠滴眼液)及安慰剂(0.1%玻璃酸钠)治疗1周。在治疗前后均使用活体共聚焦显微镜检查,观察记录角膜表层上皮细胞形态,并对图像进行分析、记录。结果:治疗前四组之间角膜表层上皮细胞密度、角膜表层上皮细胞面积比较均无统计学差异(P>0.05);治疗后,治疗组较对照组单位面积的角膜表层上皮细胞数明显增多,角膜表层上皮细胞面积明显减少,差异有统计学意义(P<0.05);治疗组间,小牛血去蛋白提取物眼用凝胶组较其他两组单位面积的角膜表层上皮细胞数明显增多,角膜表层上皮细胞面积明显减少,差异均有统计学意义(P<0.05);其他两组之间比较差异无统计学意义(P>0.05)。结论:三种角膜修复剂在治疗角膜损伤时均有明显效果,其中以小牛血去蛋白提取物眼用凝胶疗效最优。Objective： To investigate and evaluate the effects of three different eye drops on corneal epithelial wound in dry eye . Methods： 211 eyes of 160 patients with corneal epithelial wound in dry eye were assigned in the study. They were randomly divided into four groups according to different eye drops： Group ACRecombinant Bovine Basic Fibroblast Growth Factor Eye Drops） , Group B（ Deproteinized Calf Blood Extract Eye Gel） ,Group C（0. 3% Sodium Hyaluronate Eye Drops） and Group D（0. 1% Sodium Hyaluronate Eye Drops）. Group D served as a control while the other groups served as treatment. The count and area of corneal epithelium wound were assessed by laser con-focal microscope examination on the prior treatment and 1 weeks after treatment. Results： Before treatment, the count and area of corneal epithelium wound was no significant deference in the four groups（P〉0. 05）. 1 week af-ter treatment , the count of corneal epithelium in the treatment groups showed significant improvement compared with Group D; the area of corneal epithelium in the treatment groups showed significant decrease compared with Group D（P〈. 05）. Besides, Group B was more significant than the other treatment groups（P〈0. 05）; group A showed no significant compared with group C（P〉0. 05）. Conclusion： The study suggests that Recombinant Bovine Basic Fibroblast Growth Factor Eye Drops, Deproteinized Calf Blood Extract Eye Gel and 0. 3% Sodium Hyaluronate Eye Drops are effective in the treatment of corneal epithelial wound in dry eye. Besides, Deproteinized Calf BloodExtract Eye Ge is the most effective of the three eye drops.
