IL28B基因多态性与聚乙二醇干扰素ɑ治疗HBeAg阳性慢性乙型肝炎24周疗效的关系

摘　　要：目的探讨IL28B基因多态性（SNP）与聚乙二醇干扰素ɑ（Peg IFNα）24周疗程治疗HBe Ag阳性慢性乙型肝炎（CHB）疗效的相关性。方法采集398例接受Peg IFNα24周疗程治疗HBe Ag阳性CHB患者的外周血,采用直接测序法检测IL28B基因3个SNP位点rs12980275、rs12979860、rs8099917的多态性。应答定义：谷丙转氨酶（ALT）复常、HBV DNA〈500IU/m L、HBe Ag血清转换。结果治疗总应答率为42.46%（169/398）,HBV基因B型52.01%（207/398）,C型47.99%（191/398）。IL28B 3个SNP位点的基因型在应答组与无应答组中的分布无显著性差异（P〉0.05）,rs12980275 AA型与非AA型应答对比（OR 0.916,95%CI 0.460-1.825,P=0.804）,rs12979860 CC型与非CC型应答对比（OR 0.921,95%CI0.442-1.921,P=0.826）,rs8099917 TT型与非TT型应答对比（OR 0.921,95%CI 0.442-1.921,P=0.826）。结论 IL28B的SNP与聚乙二醇干扰素24周的治疗应答无显著相关。Objective To investigate whether IL28B polymorphisms could affect the response of 24 weeks treatment of Peginterferon alpha（Peg IFNα） in chronic hepatitis B HBe Ag-positive patients. Methods A total of 398 hepatitis HBe Ag-positive patients with 24 week treatment of Peg IFNα were enrolled in the study.Genotype analysis was performed for IL28B rs12980275,rs12979860 and rs8099917 using the direct sequencing method.Response was defined as cases showing normal aminotransferase（ALT） levels,HBV DNA level〈 500IU/m L and HBe Ag seroconversion. Results The patients were infected with hepatitis B virus（HBV） genotype B（52.01%） and C（47.99%） With a total response rate of 42.6%.For the three SNPs,there were not significant differences between the response（R） and non- response（NR） groups in genotype distribution（P 〉0.05）. IL28 B genotype at rs12980275 with R for AA versus N-AA（OR 0.916,95%CI 0.460-1.825, P=0.804）, rs12979860 CC versus N-CC（OR 0.921,95%CI 0.442-1.921,P=0.826） and rs8099917 TT versus N-TT（OR 0.921,95%CI 0.442-1.921,P=0.826）. Conclusion Polymorphisms in IL28 B were not significant differences of response with 24 weeks treatment of Peg IFNα in HBe Agpositive CHB patients.
