摘 要:目的利用受试者工作特性(ROC)曲线评价^(18)F-FDG PET/CT肿瘤代谢显像对肺癌病灶的检测能力,并探讨肿瘤/对侧肌肉的最大标准化摄取值(SUVmax)比值(TNR)、肿瘤/肝脏的SUVmax比值(TLR)对肺部良、恶性病灶诊断的临床价值。资料与方法回顾性分析因肺占位性病变行^(18)F-FDG PET/CT肿瘤代谢显像的患者82例,以病理为"金标准",通过t检验及ROC曲线研究^(18)F-FDG PET/CT肿瘤代谢显像对肺占位性病变的诊断价值。结果 TNR、TLR在肺部良、恶性病变中的差异有统计学意义(P<0.01)。TNR、TLR诊断肺占位性病变ROC的曲线下面积分别为0.927、0.943。TNR最佳诊断阈值为6.87,敏感度为86.36%,特异度为62.50%,准确度为81.71%。TLR最佳诊断阈值为2.86,敏感度为90.91%,特异度为68.75%,准确度为86.58%。结论 TNR、TLR在^(18)F-FDG PET/CT肿瘤代谢显像鉴别肺良、恶性病变有一定的临床价值,但是对于^(18)F-FDG摄取不典型的患者,建议结合临床相关资料进行诊断。Purpose To evaluate the role of ^18F-FDG PET/CT tumor metabolic imaging in the detection of lung cancer lesions, using receiver operating characteristic(ROC) curves analysis and investigate the diagnostic value of TNR and TLR in lung lesions. Materials and Methods A total of 82 patients who were confirmed with lung space-occupying lesionsby ^18F-FDG PET/CT tumor metabolic imaging were retrospectively analyzed. Based on the pathological gold standard, evaluate the diagnostic value of ^18F-FDG PET/CT tumor metabolic imaging in lung space-occupying lesions by t-test and ROC curve. Results There was a significant difference of TNR and TLR between benign and malignant lesion(P0.01). The AUC on ROC curve of TLR ratio was 0.943 in the lung lesions while the TNR was 0.927. The sensitivity, specificity and the accuracy were 86.36%, 62.50% and 81.71%, respectively with the TNR cutoff of 6.87. The sensitivity, specificity and the accuracy were 90.91%, 68.75% and 86.58%, respectively with the TLR cutoff of 2.86. Conclusion ^18F-FDG PET/CT tumor metabolic imaging is useful for differentiating lung space-occupying lesions. But for cases of atypical ^18F-FDG uptake, it is suggested to combine with clinical information.
