经肛门括约肌间切除术治疗超低位直肠癌的临床疗效分析

摘　　要：为探讨经肛门括约肌间切除术治疗超低位直肠癌患者的临床疗效，回顾分析2011年1月至2014年6月于我院行经肛门括约肌间切除术的90例T1和T2期超低位直肠癌患者资料，其中30例术中切除全部内括约肌（A组），30例切除部分内括约肌（B组），30例保留部分齿状线（c组）。参照Williams分级标准评价3组患者术后肛门功能，并进行对比分析。结果显示，术后3、6、12个月时，B组和c组的肛门功能良好率均优于A组，差异均有统计学意义，P〈0．05。结果表明，经肛门括约肌问切除术治疗T1和T2期超低位直肠癌安全有效，尽可能地保留部分内括约肌和齿状线对改善术后肛门功能具有重要意义。In order to explore the clinical effect of via anus intersphincteric resection in the treatment of su- pra-low rectal carcinoma,author retrospectively analysed the data of 90 cases with T1 stage and T2 stage supra-low rectal carcinoma who had received above procedure in author＇s hospital from Jun. 2011 to June 2014, included group A（ n =30, received whole internal sphincter resection）, group B（ n =30, partly resec- ting internal sphincter）, and group C （ n = 30, preserved part of dental line） ; evaluated the postoperative function of anus among the three groups according Williams grading criterion, then compared and analysis them.As results,at 3,6 and 12 months after operative in good rate of anal function group B and C was all superior to group A,there was statistical difference（ P 〈0.05）.Results show that via anus intersphincteric resection for T1 and T2stage supra-low rectal carcinoma is safe and effective,and as possibly as retain part of internal sphincter and dental line so that has important significance to improve postoperative function of anus.
