颞下颌关节灌洗术对颞下颌关节紊乱Wilkeslll期患者临床症状及关节盘形态的影响

摘　　要：目的分析研究颞下颌关节灌洗术对颞下颌关节紊乱Wilkeslll期患者临床症状及关节盘形态的影响。方法选取2014年4月至2015年9月收治的颞下颌关节紊乱Wilkeslll期患者106例,所有患者均给予颞下颌关节灌洗术治疗,观察患者治疗前后临床症状及关节盘形态变化情况。结果治疗后患者各项临床症状均较治疗前明显改善,总关节盘变形率较治疗前明显增加,而关节渗液发生率则较治疗前明显降低,差异均有统计学意义(P<0.05)。结论颞下颌关节灌洗术可明显改善颞下颌关节紊乱Wilkeslll期患者临床症状,降低关节腔渗液,但对关节盘形态恢复无明显作用,甚至有加重趋势。Objective To study the effect of lavage of temporomandibular joint on clinical symptoms and shape of articular disc in patients with Wilkes stage Ⅲ of temporomandibular joint disorder.Methods 106 patients with temporomandibular joint disorder at Wilkes stage Ⅲ during April 2014 to September 2015 were included in this study,all patients were given with lavage of temporomandibular joint for treatment,and the clinical symptoms and morphological changes in disc were observed before and after the treatment.Results The clinical symptoms of patients were significantly improved after treatment,the total incidence of disc deformation was significantly increased compared with those before treatment,and the incidence of joint effusion was significantly lower than that before treatment,and the difference was statistically significant（ P〈0.05）.Conclusion The lavage of temporomandibular joint can significantly improve the clinical symptoms of patients with temporomandibular joint disorder at Wilkes stage Ⅲ,and reduce the effusion of joint cavity,but there is no significant effect on the shape of joint disc,even increase its trend of worsening.
