氟桂利嗪联合头痛宁胶囊对偏头痛患者的疗效及对患者血清MMP-9、TNF-α及LPA的影响

摘　　要：目的探讨氟桂利嗪联合头痛宁胶囊治疗偏头痛患者的疗效及对患者血清基质金属蛋白酶-9(MMP-9)、肿瘤坏死因子-α(TNF-α)及溶血磷脂酸(LPA)水平的影响。方法选取2015年1月至2016年1月治疗的120例偏头痛患者,随机将其分为头痛宁组(盐酸氟桂利嗪+头痛宁治疗)、常规组(盐酸氟桂利嗪治疗)各60例,4周为一个疗程。观察两组头痛发作频次、头痛发作持续时间、头痛发作程度评分;评价两组患者临床疗效和不良反应;检测并比较两组患者治疗前、治疗后的血清MMP-9、TNF-α及LPA水平。结果治疗后,头痛宁组患者的头痛发作频次、头痛发作持续时间、头痛发作程度评分均低于常规组,差异具有统计学意义(P<0.05),头痛宁组患者的血清MMP-9、TNF-α及LPA水平均低于常规组,差异具有统计学意义(P<0.05);治疗后,头痛宁组患者的总有效率95.00%(57/60)高于常规组的81.67%(49/60),差异具有统计学意义(P<0.05)。结论氟桂利嗪联合头痛宁胶囊治疗偏头痛患者的疗效肯定,能够进一步降低患者血清MMP-9、TNF-a及LPA水平,提高临床效果。Objective To investigate the curative effect of Flunarizine combined with Tou Tong Ning Capsule in the treatment of patients with migraine,and effect on serum matrix metalloproteinase- 9（ MMP- 9）,tumor necrosis factor alpha（ TNF- α） and lysophosphatidic acid（ LPA）. Methods From January 2015 to January 2016,120 cases with the diagnosis of migraine were treated in our hospital. The SAS software was used to divide randomly these cases into Toutongning group（ flunarizine hydrochloride ＋ Toutongning treatment） and conventional group（ flunarizine hydrochloride）. 60 cases were in each group. Each patient was treated for 4 weeks for a course. The frequency of headache attack,the duration of headache attack and the degree of headache attack were observed. The clinical efficacy and side effects of the two groups were evaluated. Serum levels of MMP- 9,TNF- α and LPA were measured before and after treatment in both groups. Results After treatment,the headache frequency,headache duration,headache severity scores in patients of Toutongning group were lower than the normal group,the difference was statistically significant（ P〈0. 05）. The serum MMP- 9,TNF- α and LPA levels of patients of Toutongning group were lower than the normal group,the difference was statistically significant（ P〈0. 05）. After treatment,the total efficiency in Tou Tong Ning group was 95. 00%（ 57 /60）,which was higher than the conventional group 81. 67%（ 49 /60）,the difference was statistically significant（ P〈0. 05）. Conclusion Flunarizine combined with Tou Tong Ning Capsule in the treatment of migraine patients had definite curative effect. This method can further reduce the level of serum MMP- 9,TNF- α and LPA,improve the clinical effect.
