摘  要:目的:评价微波凝固术治疗鼻咽癌肺转移的临床价值。方法:回顾性分析2004年5月至2012年7月对98例鼻咽癌肺转移瘤患者168个病灶行微波凝固治疗的临床疗效,经微波凝固治疗1个月后行胸部CT增强扫描评价肿瘤治疗效果。结果:98例患者的168个病灶在治疗1月行CT复查评价疗效,完全缓解50个,部分缓解44个,稳定44个,进展30个,有效率达82.1%。全部病例术后均未出现严重并发症。患者1年、2年、3年的总生存率分别为46.7%,28.3%,13.4%。患者转移灶最大直径大小及累计转移灶的多寡对患者生存有显著影响。结论:微波凝固有较好的长、短期疗效,而且安全可靠,副作用小,是治疗鼻咽癌肺转移的有效方法之一。肺转移灶肿瘤最大径、累计转移灶的多寡均直接影响患者的总生存。Objective To analyze the clinical value of percutaneous microwave coagulation therapy(PMCT) in the treatment of lung metastases caused by nasopharyngeal carcinoma. Methods 98 patients(hospitalized from May 2004 to July 2012) with lung metastasis of nasopharyngeal carcinoma treated by PMCTwere investigated retrospectively in this study. The results were evaluated by the double-phase CT scanning.Results All patients received chest CT scan 1 month after the procedure.Among 168 lesions in 98 cases,CRwas 50,PR 44,SD 44 and PD 30.The effective rate was 82.1%。The overall survival rates for pulmonary metastatictumor patients in 1 year,2 years,and 3 years were 46.7 %,28.3%,and 13.4% respectively. Masmium metasisdiameter and accumulated metasis amount have obvious effect on patients′ survival. Conclusion PMCT waseffective in the short- and long- term treatment. It is safe and reliable in the treatment of lung metastasis caused bynasopharyngeal carcinoma.The maximum diameter and the accumulated metasis amount in lung metastatic lesionhad an important effect on overall survival.
