摘　　要：目的分析原发性甲状腺淋巴瘤(PTL)的超声影像特征及其相应的病理学表现。方法采用彩色多普勒超声扫描并复阅病灶病理及免疫组化染色切片,对照分析29例(男3例,女26例,年龄50~83岁,中位年龄67岁)不同病理类型PTL的超声影像表现及病理特征。结果 29例PTL中15例甲状腺双侧叶均受累,13例单侧叶受累,1例峡部受累。超声表现为9例呈单纯弥漫型改变,12例呈甲状腺弥漫性病变背景下合并多发低回声结节型改变,7例呈肿块型改变,1例呈甲状腺双侧叶多发低回声结节。29例PTL中有21例(72.4%)伴周围颈部区域淋巴结肿大。不同病理类型的PTL对应不同的超声表现类型,黏膜相关淋巴组织结外边缘区B细胞淋巴瘤超声影像多表现为弥漫型(3/4);混合型淋巴瘤均表现为弥漫型伴多发结节型(7/7);弥漫大B细胞型非霍奇金淋巴瘤超声影像表现多样,其中弥漫型5例,弥漫型伴多发结节4例,肿块型7例;富于T细胞/组织细胞的大B细胞淋巴瘤表现为弥漫型(1/1)。结论不同病理类型的PTL具有一定特征性的超声影像特点。Objective To study the ultrasonographic features and their corresponding pathological appearances of primary thyroid lymphoma （PTL）. Methods The ultrasonographic features of 29 cases with PTL were analyzed and compared with the result of pathology by using the color Doppler ultrasound scanner and the histological and immunohistochemical staining sections. In the 29 cases, there were 3 males and 26 females with a median age of 67 years old, which aged from 50 to 83 years. Results In 29 cases, 15 patients had bilateral thyroid involvement, 13 patients had unilateral thyroid involvement and 1 patient had isthmus involvement. The sonographic findings of primary thyroid lymphoma could be classified into：diffuse type （9/29）, diffuse type with multiple nodular type （12/29）, mass type （7/29）, and multiple nodular type （1/29）. In 29 cases, 21 cases （72.4%） have cervical lymph node enlargement. Each pathologic type of PLT had its specific sonographic appearances. The mucosa associated lymphoid tissue node extranodal marginal zone B-cell lymphoma showed diffuse type on ultrasonography （3/4）;The mixed type lymphoma showed diffuse type with multiple nodules （7/7）;The diffuse large B-cell non Hodgkin’s lymphoma showed diversity on ultrasonography （5 cases of diffuse type, 4 cases of diffuse type with multiple nodules and 7 cases of mass type）;And the full of T cell/tissue cells large B cell lymphoma showed diffuse type. Conclusions Different pathological types of PTL have their specific characteristics on sonography.
