依托咪酯脂肪乳-丙泊酚混合液在老年无痛胃镜中的临床应用效果

摘　　要：目的:探讨依托咪酯脂肪乳-丙泊酚混合液在老年无痛胃镜检查中的麻醉有效性及安全性。方法:选择ASAⅠ~Ⅱ级门诊胃镜检查患者180例,按照随机数字表法将患者分为丙泊酚组(P组)、依托咪酯脂肪乳组(E组)、依托咪酯脂肪乳-丙泊酚组(E-P组),每组60例。监测记录给药前(T0)、给药后1 min(T1)、苏醒时(T2)的血压、心率(HR)、呼吸频率(RR)、苏醒时间及术中体动、呼吸抑制、心动过缓、低血压、肌阵挛的发生情况。结果:三组患者的胃镜检查时间比较,差异无统计学意义(P>0.05);三组患者苏醒时间比较,差异有统计学意义(P<0.05)。E-P组和E组患者给药后(T1)、苏醒时(T2)的平均动脉压(MAP)、HR、RR与给药前(T0)比较,差异均无统计学意义(P>0.05);P组患者T1时的MAP、HR、RR及T2时MAP、HR与T0时比较均明显降低,差异均有统计学意义(P<0.05)。P组患者T1、T2时的MAP、HR均明显低于E-P组、E组,差异均有统计学意义(P<0.05)。P组患者呼吸抑制、低血压、心动过缓发生率均明显高于E组和E-P组,差异均有统计学意义(P<0.05);E组患者的体动反应、肌阵挛发生率均高于P组和E-P组,差异均有统计学意义(P<0.05)。结论:依托咪酯脂肪乳-丙泊酚混合液对循环、呼吸影响较小,不良反应发生率低,可安全用于老年无痛胃镜检查。Objective：To investigate the anesthetic efficacy and safety of Etomidate Fat Emulsion and Propofol mixture in elderly patients undergoing analgesia gastroscopy.Method：180 elderly patients examinated by analgesia gastroscopy with ASAⅠ-Ⅱ of outpatient were selected,according to the random number table method,they were randomly divided into Propofol group（P group）,Etomidate Fat Emulsion group（E group）,Etomidate Fat Emulsion and Propofol mixture group（E-P group）,60 cases in each group.The BP、HR、RR and awakening time were recorded before drug injection（T0）,1 min after drug injection（T1） and palinesthesia（T2）,meanwhile,body movement,respiratory depression,bradycardia,hypotension and muscle spasm were observed.Result：The gastroscopy time of the three groups were compared,there was no statistically significant difference（P〈0.05）.The waking time of the three groups were compared,the difference was statistically significant（P〈0.05）.There were no significant differences in mean arterial pressure（MAP）,HR,RR at 1 min after drug injection（T1） and palinesthesia（T2） of E-P group and E group comparede with T0（P〈0.05）.MAP,HR in P group at T1 and T2 were significantly lower than those in E-P group and E group,the differences were statistically significant（P〈0.05）.The incidence of respiratory depression,hypotension and bradycardia in P group were significantly higher than those in E group and E-P group,the differences were statistically significant（P〈0.05）;body movement and incidence of myoclonus in E group were higher than those of P group and E-P group,the differences were statistically significant（P〈0.05）.Conclusion：Etomidate Fat Emulsion and Propofol mixture on respiratory and circulation,less effect,low incidence of adverse reactions,is safe for elderly painless gastroscopy
