摘　　要：目的 探讨在外伤性晶状体不全脱位白内障手术中应用针钩娩核法的临床效果及安全性.方法 回顾性分析白内障伴外伤性晶状体不全脱位21例（21只眼）的病历资料：患者年龄45 ～71岁,晶状体脱位范围均＜120°,术前视力均≤0.1.评估术前术中悬韧带离断范围、人工晶状体植入情况、术后视力及术后反应.结果 3例（14.29％）术中发生悬韧带离断范围扩大,但和术前相比较差异无统计学意义（x2=3.231,P=0.072）.未出现明显人工晶状体移位或核下沉等严重并发症.术后1d裸眼视力0.1～0.5,优于术前视力,差异有统计学意义（Z=-4.016,P=0.000）.术后1个月复查,未见明显人工晶状体移位.结论 在白内障伴晶状体不全脱位患者应用针钩娩核法是安全、有效的.Objective To investigate the clinical effect and safety of application of capsulotomy needle and Sinskey hook in surgery for traumatic cataract with subluxation of lens.Methods Twenty-one eyes of 21 patients aged 45-71 years with cataract and traumatic lens subluxation who underwent manual small incision cataract surgery （MSICS） were retrospectively reviewed.The nucleair all were Ⅱ-Ⅳ grade.The detaching ranges all were 〈 120° and the pre-operative visual acuity was 0.1 or less.The detaching range of suspensory ligament preoperative and intraoperation,information of intraocular lens （IOL） implantation,post-operative uncorrected visual acuity （UCVA） and inflammatory response were evaluated.Results Three （14.3%） patients had an expanded breaking range of suspensory ligament during cataract surgery,but there was no statistically significant difference compared with that of preoperation （x2 =3.231,P =0.072）.All patients underwent IOL implantation.There was no serious complication such as IOL shift,falling of nucleus and tearing of capsule.The uncorrected visual acuities of all cases were 0.1-0.5 at 1 day after surgery,which was significantly better than those of preoperation （Z =-4.016,P =0.000）.No obvious IOL shift was observed at 1 month after surgery.Conclusion MSICS assisted with capsulotomy needle and Sinskey hook in cataract with lens subluxation is safe and effective.
