摘  要:先天性腹股沟疝在儿科门诊体检中发病率较高,多数儿科体检医师未经过腹股沟区解剖学方面的疝和腹壁外科专业培训,故而在筛查中检出率较低,特别是隐匿疝难以诊断。通过对照分析,探讨腹腔镜联合开放手术视频的新教学模式,在儿科体检医师腹股沟疝筛查培训中的作用。将已确诊的60例儿童腹股沟疝患者(2~4岁)随机分为2组(每组患儿30例),对我部门参与门诊腹股沟疝筛查的体检医师(12例)进行腹腔镜联合开放的腹股沟区解剖教学,在培训前后,分别对一组儿童进行体检,记录腹股沟疝检出率,并标记体表内环口位置,以超声确定标记的准确率。通过新模式解剖教学,入组体检医师培训后的笔试成绩、腹股沟疝检出率和内环口标记准确率均高于培训前,具有统计学意义。对儿科体检医师进行新模式教学可以有效提高儿童腹股沟疝检出率。Congenital inguinal hernia was common in the outpatient department of Pediatrics examination, the majority of pediatric physician hadn't received specialized training of inguinal hernia anatomy and surgery. Therefore the screening detection rate is low, especially for the occult hernias. To explore the role of laparoscopic combined opening surgery video teaching model in the training of pediatric inguinal hernia screening. 60 children with inguinal hernia were randomly into 2 groups ( n = 30 in each group). 12 doctors of Renmin University of China Hospital outpatient department for screening of inguinal hernia trained by the laparoscopy combined with opening surgery inguinal anatomy teaching. Physical examination for a group of children were recorded before and after the training, respectively. The detection rate of inguinal hernia and the accuracy rate of marking the inner ring position were recorded. For all 12 doctors, the result of written exam, detection rate of inguinal hernia and accuracy of inner ring marking were significantly improved than those before the training. The new training can effectively improve the detection rate of inguinal hernia in children.
