腹腔镜下门奇静脉断流加脾切除术治疗肝硬化门静脉高压症的效果观察

摘　　要：目的探讨腹腔镜下脾切除+门奇静脉断流术治疗肝硬化门静脉高压症的效果。方法选取100肝硬化门静脉高压症患者,根据手术方式的不同分为观察组(n=50)和对照组(n=50),观察组采用腹腔镜下门奇静脉断流术+脾切除术,对照组采用传统开腹手术。结果 100例均成功手术。观察组手术时间长于开腹组,术中出血量、术后下床时间、平均排气时间、住院时间均少于对照组(P<0.05)。观察组患者生活质量高于对照组(P<0.05)。结论腹腔镜下门奇静脉断流术+脾切除治疗肝硬化门静脉高压症的近期疗效优于传统开腹手术。Objective To study the clinical effect of laparoscopic splenectomy combined with portal azygous vein disconnection in the treatment of liver cirrhosis portal hypertension. Methods Totally 100 cases of liver cirrhosis portal hypertension patients were selected. The patients were divided into two groups, 50 cases in each group. The observation group adopted laparoscopic portal azygous vein disconnection combined with splenectomy, the control group adopted the conventional laparotomy. Results The operation was successful. The operation time of the observation group was longer than that of the open group, The amount of bleeding, postoperative bed time, the average exhaust time and hospital stay were less than those of the control group（P〈0.05）. The quality of life in the observation group was significantly higher than that in the control group（P〈0.05）. Conclusion Laparoscopic portal azygous vein disconnection operation ＋ spleen resection for the treatment of liver cirrhosis portal hypertension recently and the curative effect is better than traditional open surgery.worthy of clinical application.
