左半结肠癌合并肠梗阻患者行肿瘤切除I期吻合术39例病例分析

摘　　要：目的探讨左半结肠癌合并肠梗阻患者行I期切除肠吻合术治疗的临床疗效。方法回顾性分析2012年1月至2016年1月收治的45例左半结肠癌合并肠梗阻患者的临床治疗资料。结果39例患者行I期切除吻合术，5例患者行左半结肠I期切除，Hartman结肠造瘘，Ⅱ期闭瘘术，1例患者行姑息性结肠造口术，手术均成功，术后仅分期手术患者中出现1例切口感染。结论在完善围手术期处理，严格掌握适应证的情况下，左半结肠癌合并肠梗阻患者可行I期切除吻合术。Objective To investigate the clinical effect of one-stage resection and anastomosis for the intestinal obstruction due to left-sided colon cancer. Methods The clinical data of 45 patients with intestinal obstruction due to left-sided colon cancer were review. Results 39 patients were treated with one-stage resection and anastomosis, 5 patients were treated by left-sided colon resection and colostomy Hartman, with closing fistula as second phase, and 1 patients underwent palliative colostomy. All operations were successful. Incision infection occurred in 1 case. Conclusions With the improvement of perioperative management and strictselection of indications, left-sided colon cancer with bowl obstruction in patients with one-stage resection and anastomosis is safe.
