乳腺X线摄影和MRI直方图在鉴别乳腺纤维腺瘤和浸润性导管癌中的价值

摘　　要：目的探索直方图技术在鉴别乳腺纤维腺瘤和浸润性导管癌中的价值。方法收集2015年1月~2015年9月在我院行乳腺X线摄影及MRI检查的女性患者22例,其中经病理证实的乳腺纤维腺瘤8例,未见明显钙化的浸润性导管癌14例。使用Image J软件计算病变区直方图的偏度、峰度、均值及中值,直方图模式,病变区密度,统计分析上述指标在良恶性病变中的差异。结果在X线摄影中,浸润性导管癌病变区直方图的密度大于纤维腺瘤(P<0.05),偏度和峰度也具有统计学差异(P<0.05);在磁共振T1WI增强图像,浸润性导管癌病变区直方图峰度、偏度和模式与纤维腺瘤有显著性差异(P<0.01),但其他指标均无明显差异。相关性分析发现在X线摄影图中,密度与灰度均值有正相关性(P<0.05);在MRI增强图像,密度与直方图峰度有强正相关性(P<0.01)。结论在鉴别乳腺纤维腺瘤和未见明显钙化的浸润性导管癌时,X线摄影直方图和磁共振增强像直方图具有一定临床价值。Objective To explore the value of histogram in distinguishing of breast fibroadenoma and infiltrating ductal carci- noma without obvious calcification. Methods 22 patients underwent breast mammography and MRI scanning were included from January 2015 to September 2015 in hosptial. 8 cases of breast fibroadenoma and 14 cases of infiltrating ductal carcinoma without obvious calcification had been proved by pathology. The skew, kurtosis, mean, median and mode of histogram as well as the den- sity of the lesion were calculated by ImageJ software. The different between benign group and malignant group of the above index were analyzed. Results In mammography, the density of the lesion from infiltrating ductal carcinoma was significantly higher than that from breast fibroadenoma （ P 〈 0.05）, skew and kurtosis between two groups had also statistical difference （ P 〈 O. 05）. In enhanced T1 weighted MRI, the skew, knrtosis and mode of histogram between two groups had significantly statistical difference （ P 〈 0.01 ）, though no difference in other index. The positive correlation between the density and mean in mammog- raphy （ P 〈0.05） and strong positive correlation between the density and kurtosis in enhanced MRI （ P 〈0.01 ） were found by correlation analysis. Conclusion The histogram of breast mammography and enhanced MR! has certain clinic value in distinguis- hing of breast fibroadenoma and infiltrating ductal carcinoma without obvious calcification.
